- Exhibit C - PAST PERFORMANCE REFERENCE QUESTIONNAIRE Exhibit B - SUBCONTRACTOR INFORMATION AND CONSENT FORM Exhibit A - PERFORMANCE RELEVANCY QUESTIONNAIRE Solicitation Document Full Food Service The USPFO for Wisconsin is issuing a Request for Proposal (RFP) to award a Single Indefinite Quantity Indefinite Delivery firm - fixed-price contract for Full Food Service in accordance with the Performance Work Statement and Performance Standards within the solicitation document attached. The requirements of this contract include providing all qualified personnel, materials, supervision, and quality control necessary, to perform food service support operations including breakfast, lunch and dinner meals on an as needed basis. The Period of Performance is one base year and two option years. The estimated price range for the entire period of performance is between $1,000,000 and $5,000,000. The location of the work is at the Regional Training Institute (RTI), Fort McCoy, WI. The North American Industry Classification System (NAICS) code for this work is 722310 Food Service Contractors. This is a total small business set- aside. The small business size standard is $38.5 million average annual revenue. The selection process will be conducted in accordance with FAR Part 15, Source Selection Procedures. Proposals will be evaluated on Past Performance, Technical, and Price. Prospective Offerors must submit a written past performance and technical proposal and price proposal as well as Exhibits A, B, and C where appropriate. Refer to Proposal Instructions on page 50 of the solicitation for detailed instructions on submission requirements. An organized site visit will be conducted on 14 May 2018 at 10:00 a.m. Central Standard Time (CST) at the Regional Training Institute, 90 South 10th Avenue, Fort McCoy, Wisconsin. If you plan to attend the site visit, email Lisa Braund at lisa.m.braund.mil@mail.mil no later than 10:00 a.m. CST, 11 May 2018 for further instructions concerning base access. Interested contractors are encouraged to attend and shall follow base access requirements. Questions regarding this solicitation shall be in writing and sent via e-mail only to Lisa Braund at lisa.m.braund.mil@mail.mil no later than Thursday May 17th 2018 by 2:00 p.m. CST. All questions will be answered electronically and in writing through a solicitation amendment. Proposals shall be sent via e-mail ONLY to Lisa Braund at lisa.m.braund.mil@mail.mil no later than Wednesday, May 23rd, 2018, 2:00 p.m. CST. We recommend submitting your proposal prior to the closing date to allow enough time to ensure the entire proposal is received on time to be considered for award.
