- FA4803-18-R-0004, Attachment 5, Notification of Compliance with Contract Insurance Requirements FA4803-18-R-0004, Attachment 4, Past Performance Questionnaire (PPQ) FA4803-18-R-0004, Attachment 3, Quality Assurance Surveillance Plan (QASP) FA4803-18-R-0004, Attachment 2, Wage Determination 15-4439 (Rev.-5) FA4803-18-R-0004, Attachment 1, Performance Work Statement (PWS) FA4803-18-R-0004, Request for Proposal for Operation and Maintenance of Waste Water Treatment Plant and Waste Water Disposal Shaw AFB intends to award a firm-fixed price contract, base and four (4) one-year option periods for Operation and Maintenance of Waste Water Treatment Plant and Waste Water Disposal for Shaw Air Force Base and Wateree Recreation Area, South Carolina. Solicitation number FA4803-18-R-0004 is issued as a Request for Proposals. Award will be made from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. This effort will be conducted under FAR Parts 12 and 13 procedures allowed by FAR 13.5. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-64, Defense Federal Acquisition Regulation Supplement Change Notice (DCN) 20121231 and Air Force Acquisition Circular (AFAC) 2012-1107. This action is a100 percent set-aside for Small Business (SB). The North American Industry Classification System (NAICS) code is 221320 with a small business size of $20.5M. The contractor shall furnish all personnel, vehicles, tools, supplies and equipment required to Operation and Maintenance of Waste Water Treatment Plant (WWTP) and Waste Water Disposal at Shaw Air Force Base (SAFB) located in Sumter County SC and the Wateree Recreation Facility located in Kershaw County SC. Operations and maintenance shall be in accordance with South Carolina Department of Health and Environmental Control (DHEC), National Pollutant Discharge Elimination System (NPDES) Permit Numbers SC0024970 and SC0044440, the Clean Water Act, the Pollution Control Act, the plant's operation and maintenance (O & M) manuals and all applicable federal, state and local laws, regulations, standards and commercial practices as described in the performance work statement (PWS) with the focus on safety and customer service. The contractor is encouraged and expected to use innovative approaches to efficiently and effectively accomplish contract requirements in a timely manner at reduced costs in a way which fosters pride and ownership in the work performed. Historical chemical usage is listed in Appendix A, Historical Data. All chemicals must be coordinated through Hazmart (building 231) for entry into Enterprise Environmental, Safety and Occupational Health Management Information System (EESOH-MIS). The Government will not pay for any material provided nor the return of the data provided. The closing date and time for submission of offers will be contained in the solicitation package. Prospective awardees MUST be registered in the System of Award Management (SAM) at http://www.sam.gov.
