Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 in conjunction with FAR Part 13, as supplemented with additional information included in this notice. This combined synopsis/solicitation is issued as a Request For Quotations (RFQ). NAICS: 238990 All Other Specialty Trade Contractors. Size Standard: $15.0M This combined synopsis/solicitation is being issued as a Service Disabled Veteran Owned Small Business Set-Aside. Public Law 109-461, 38 USC Sec. 8127 provides SDVOSB set aside authority to the Department of Veterans Affairs. Businesses must be registered and verified in the VIP database at Vendor Information Pages (VetBiz). Award will be made to the responsive offeror whose offer is the lowest priced and conforming to the solicitation considering only price and any price-related factors specified in the solicitation. Place of Performance: VA Loma Linda Healthcare System VA Loma Linda Medical Center 11201 Benton Street Loma Linda, CA 92357 Period of Performance: 1 year (Base Year) and Four Option Years The Department of Veterans Affairs Loma Linda Healthcare System has a requirement for herbicide and weed services per the Statement of Work. All services will be performed in strict adherence to the Statement of Work (attached). PST. Attendance is NOT mandatory, but highly encouraged at the VA Loma Linda Healthcare System. Participants are asked to meet outside of the front main entrance at the VA Loma Linda Medical Center in the lobby near the Starbucks. Please RSVP for the site visit via email to Contract Specialist, James Barber at james.barber@va.gov NLT Monday May 21, 2018 at 1:00 P.M. PST. The following clauses apply: FAR 52.212-1 - Instructions to Offerors applies to this acquisition Provision Addenda: All quotes shall include the following minimum information: Cage Code, Duns Number, Taxpayer I.D. No., Payment Terms, and/or Discount Terms, Offerors Reps and Certs, Point of Contact, Phone, Fax, Email and Proposed Delivery Schedule. Prospective vendors must be registered in the System for Award Management (SAM) database prior to contract award. Lack of SAM registration shall be a determining factor for contract award. Prospective vendors should visit the SAM website at http://www.sam.gov to register. FAR 13.106-2 Evaluation of Quotations or Offers - Commercial Items: Contract award will be made to the responsive offeror whose offer is the lowest priced and conforming to the solicitation considering only price and any price-related factors specified in the solicitation.
