- Attachment A- Statement of Work Attachment F - Pricing Worksheet Attachment G - Past Performance Questionnaire Attachment C - FAR 52.212-5 Contract Terms and Conditions Attachment B - FAR 52.212-3 Offeror Representations and Certifications Attachment E - Commitment to Protect NPI Form FDA 3398 Request for Quote 1193604 Attachment D - FDA 1335 Personal Security Clearance Requirements BACKGROUND The Food & Drug Administration (FDA), Office of Criminal Investigations (OCI), is responsible for conducting and coordinating agency criminal investigations relating to violations of the Federal Food, Drug, and Cosmetic Act, Federal Anti-Tampering Act, Title 18 offenses and other areas over which FDA has jurisdiction. The special agents of the OCI are charged with investigating alleged criminal violations within FDA's jurisdiction. These violations include counterfeit, unapproved and illegal drugs or medical devices; product substitution; health fraud and health care fraud and application fraud amongst other violations. It is the FDA's intent to award a contract using Small Business Set-Aside procedures, with a performance period consisting of a base period of one (1) year base with four (4) one (1) year options. NOTICE OF COMBINED SYNOPSIS/SOLICITATION This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This is a request for quotations (RFQ) for commercial items in accordance with (IAW) the procedures of FAR Part 12 - Acquisition of Commercial Items, and FAR Part 13.5 - Simplified Procedures for Certain Commercial Items. NOTICE OF FAC This solicitation document incorporates provisions and clauses in effect through Federal Acquisition Circular FAC 2005-97, effective January 24, 2018. FAR provisions and clauses referenced in this RFQ can be found on the following website: www.acquisition.gov HHSAR provisions and clauses referenced in this RFQ can be found on the following website: https://www.hhs.gov/grants/contracts/contract-policies-regulations/hhsar/index.html NOTICE OF NAICS/SET-ASIDE This requirement is being competed as a 100% Woman Owned Small Business Set-Aside under North American Industry Classification System (NAICS) Code 541612, "Human Resources Consulting Services" ($15.0 million size standard).   CONTRACT REQUIREMENTS See (Attachment A) for the Statement of Work for a full description of the supplies/services to be acquired. CONTRACT TYPE The Government anticipates awarding a Labor Hour type order. QUESTION SUBMITTAL INSTRUCTIONS Offerors may submit questions/clarifications in reference to any areas of the RFQ. Questions/clarifications must be submitted via email to Food and Drug Administration/OAGS: Haritha Raja, Contract Specialist, at haritha.raja@fda.hhs.gov, and Michelle Creenan, Contracting Officer, at michelle.creenan@fda.hhs.gov. The subject line of the email must read: Contractor Questions, OCI Employment Consulting Services, RFQ No. 1193604. Questions/clarifications submitted by any other means will not be accepted. Questions/clarifications must identify the applicable section of the RFQ. Questions/clarifications will be answered in an amendment to the solicitation. Questions/clarifications will not be discussed over the phone. All questions/clarifications must be submitted by 1:00 PM Eastern Time on May 21, 2018. Questions submitted after the due date may not be answered. Offerors will receive a confirmation email within two (2) business days that their questions were received. If an offeror does not receive a confirmation email within two (2) business days, it is their responsibility to follow up with the contract specialist to ensure their questions were received. QUOTATION SUBMITTAL DUE DATE Quotations are due no later than 11:00 PM Eastern Time on June 1,2018. Quotations must be submitted via email to Food and Drug Administration/OAGS: Haritha Raja, Contract Specialist, at haritha.raja@fda.hhs.gov, and Michelle Creenan, Contracting Officer, at michelle.creenan@fda.hhs.gov. The subject line of the email must read: OCI Consulting Services, RFQ No. 1193604. If submitting multiple emails, the subject line must clearly identify the total number of emails and which sequence the submission is. All emails/quotations must be received by the due date and time.
