Description

Eastern Colorado Health Care System Boiler Water Treatment Services This is a Sources Sought Notice (NOT A SOLICITATION). The Eastern Colorado Health Care System is seeking potential sources to provide Ground Ambulance service. This service may be awarded as 2 separate contracts if needed; one for service in the Denver Metro Area and one for service outside the Denver Metro Area Please direct all questions to ian.boettcher@va.gov General Requirements: Contractor shall furnish all labor, materials, insurance, licenses and equipment to provide safe and comfortable ambulance litter/stretcher transportation on an as-needed basis to beneficiaries of the Rocky Mountain Regional VA Medical Center (RMR) throughout the Denver Metro area and our outlying Community Based Outpatient Clinics (CBOCs) in Colorado. The vendor selected shall be ready to start on the day of award as a fully capable entity with all equipment required up and ready, with trained and certified personnel prepared to respond as directed by RMR as what is expected. Services to be provided seven (7) days a week, as detailed further below, including holidays, to and from locations (i.e. clinics, ward locations, inpatient floors, ICU, patient homes, hospitals, pre-approved fee basis appointments) designated by the RMR, RMR Beneficiary Travel Office, in accordance with all terms, conditions, provisions and schedules set forth in this solicitation. Contractor shall assist the patient as needed to access the vehicle for transportation. This may include, but is not limited to bed to stretcher or stretcher to bed transfer when necessary. B. The following healthcare facilities are under the jurisdiction of Rocky Mountain Regional VA Medical Center (13611 Colfax Ave, Aurora, CO 80045) and will be included in contract: Alamosa Outpatient Clinic, 622 Del Sol Drive, Alamosa, CO 81101 Burlington Outpatient Clinic, 1177 Rose Avenue, Burlington, CO 80807 Colorado Springs Outpatient Clinic, 3141 Centennial Blvd. Colorado Springs, CO. 80907 Denver Outpatient Clinic, 1055 Clermont Street, Denver, CO 80220 Rocky Mountain Regional VA Medical, 13611 Colfax Ave, Aurora CO. 80045 Aurora Outpatient Clinic, 13701 E Mississippi Ave, Gateway Medical Bldg. #200, Aurora, CO 80012 Jewell Outpatient Clinic, 14400 E Jewell Ave, Aurora, CO 80012 La Junta Outpatient Clinic, 1100 Carson Ave., Suite 104, La Junta, CO 81050 Lakewood Outpatient Clinic, 155 Van Gordon Street, Suite 395, Lakewood, CO 80225 Lamar Outpatient Clinic, 1401 South Main St. Suite B, Lamar, CO 81052 Pueblo Outpatient Clinic, 4776 Eagleridge Circle, Pueblo, CO 81008 Salida Outpatient Clinic, 920 Rush Drive, Salida, CO 81201 All interested firms shall submit a response demonstrating their capabilities regarding the above requirement. As stipulated in FAR 15.201, responses to this notice are not considered offers and cannot be accepted by the Government to form a binding contract. No solicitation exists; therefore, do not request a copy of the solicitation. The decision to solicit for a contract shall be solely with the Government discretion. Firms responding should indicate whether they are, or are not, a small business, a socially and economically disadvantaged business, veteran owned small business/service disabled veteran owned small business or a woman owned business. The general definition of a small business is one that is independently owned and operated, is not dominated in the field of operation in which it is proposing on Veterans Administration contracts and with its affiliate. NAICS code to be used for this acquisition is 621910 ($15.0M). The Government reserves the right to consider all types of small business set-asides based upon responses hereto for any subsequent acquisition. Respondents are further requested to indicate their status as a foreign-owned/foreign-controlled firm and any contemplated use of foreign national employees on this effort. Any information submitted by respondents to this sources-sought synopsis is voluntary. This sources-sought notice is not to be construed as a commitment by the Government, nor will the Government reimburse any costs associated with the submission of information in response to this notice. Respondents will not individually be notified of the results of any Government assessments. The Government s evaluation of the capability statements received will factor into whether any forthcoming solicitation will be conducted as a full and open competition or as a set-aside for small businesses, or any small business designation (e.g. SDVOSB, HUB Zone, 8(a), WOSB, VOSB, etc). All prospective vendors must be registered and current within the System for Award Management (SAM). Visit www.sam.gov for details.
