- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. SYNOPSIS: The Department of Veterans Affairs, VISN 6 Network Contracting Office, on the behalf of Durham VA Medical Center has a need for Multifix Radiation Therapy Immobilizer equipment Brand name or Equal to CIVCO Multifix. This is a brand name or equal requirement which means that items must meet the salient physical, functional, and performance characteristics of brand name product. This requirement is for new equipment only; no remanufactured or "gray market" items. All items must be covered by the manufacturer's warranty. All offerors must be authorized distributors of the equipment. SOLICITATION: See attached solicitation 36C24618Q0321. This solicitation has been set-aside for SDVOSBs. Offeror must be registered on SAM website and VetBiz (VIP). Quotations shall include proposed unit prices, proposed extended prices, any prompt payment discount terms, any shipping charges, company name, and point of contact, address, phone number, tax identification number, and DUNS number. The Government shall consider quotes received by 10:00 AM Eastern Standard Time, May 30, 2018. Quotes are to be e-mailed to dori.dabbs2@va.gov.
