This announcement constitutes the only solicitation; are being requested, and a written solicitation document will not be issued. This solicitation is a "request for quotations (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-97. The associated North American Industrial Classification System (NAICS) code for this procurement is 332999, with a small business size standard of 750 employees. VA Hudson Valley Healthcare System Montrose Campus at 2094 Albany Post Road is seeking to purchase Brand Name Only TBJ Incorporated Triple Bowl decontamination work sink for installation and delivery to FDR SPS department. All interested companies shall provide quotation(s), for the following: Supplies Decontamination Sink Specifications T304 stainless steel construction Heliarc welded construction All exposed welds ground and polished to #4 finish (3) 22 x 18 x 10 ½ deep sink bowls with etched gallon indicator markings in each bowl Sink bowls will be centered leaving approximately 18 of counter space on left side and 18 on the right side (1) Deck mounted hot/cold water faucet at left sink with detergent line hole and DEMA tip (1) Deck mounted hot/cold water combination spray rinse / faucet at center sink with detergent line hole and DEMA tip (1) Deck mounted RO/DI pure water faucet at right sink (1) Deck mounted Selecta compressed air pistol at right sink (1) Deck mounted Selecta water pistol at left sink (1) Set of (8) pistol attachments for Selecta pistols (4) 1 ½ x 16 gage wall stainless steel tubular legs with adjustable leveling feet 4 high back splash Raised marine edge for retaining water on work surface Overall dimensions: 30 W x 108 L Push-button height adjustment with programmable settings 8 travel range (3) Flexible drain bellows Deck mounted Eye Wash Automatic sink bowl fill and detergent proportioning system with push-button pre-set fill levels, temperature monitoring and data reporting (documents and reports number of fills, water temperature, and detergent volume via USB drive) Heated sink bowl with digital thermostat control 4-outlet Tubing flush manifold Hydro-force rinse system with pump, adjustable timer, push-button control and on/off switch Installation responsibilities Receiving of new sink Complete removal of old sink Complete installation of new sink, including packing materials, plumbing, electrical connections. All removal and installation to be performed over a weekend. Drawings to be signed off on Services Equipment delivery, as well as pull out old sink & install new sink required. The delivery/task order period of performance is 05/31/2018-07/31/2018. Delivery shall be provided no later than 15 days after receipt of order/award of contract. Delivery terms Destination. All software licensing, warranty and service associated with the equipment/system shall be in accordance with the OEM terms and conditions**. This is an open-market combined synopsis/solicitation for TBJ Incorporated Triple Bowl decontamination work sink with installation as defined herein. The government intends to award a contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all bids must include a statement regarding the terms and conditions herein as follows: "The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."
