- Statement of Work GE Equipment Relocation This is not a request for quote, the government is currently conducting market research with the intent to sole source to the manufacture. If interested please send a one page capability statement with information pertaining directly to the relocation of radiology equipment to andrew.taylor3@va.gov by May 28, 2018 by 4:00 pm. est. Scope: Contractor shall furnish all supervision, labor, materials, tools, and equipment necessary to complete the relocation of GE radiology imaging equipment and the associated accessories. Background: The Lebanon VA Medical Center (LVAMC), located at 1700 S. Lincoln Avenue, Lebanon, PA, has a requirement for the relocation of four government-owned GE radiology devices and the associated accessory equipment, as listed in Schedule A Equipment List. The equipment shall be de-installed from the present location, transported to the designated new area within the facility, and reinstalled in the new area. The contractor shall be responsible to conduct all required testing and calibration of the equipment after reinstallation, and shall ensure the equipment is restored to full operating functionality. Requirements: In accordance with the following specifications: Service Requirements: Contractor shall provide field service representatives (FSRs) to deinstall the designated equipment, per the manufacturer s deinstallation guidelines, with the intention of moving the equipment to its new space within the facility. This shall include performing disconnections from power sources. Contractor shall be responsible for the transportation and reinstallation of the same equipment into the new location as determined by a representative from Biomedical Engineering. Reinstallation shall be completed in accordance with manufacturer s specifications. Contractor shall reconnect equipment to the power sources in the new area and shall conduct all necessary work to restore equipment to full functionality. This shall include calibration, validation, and results correlation. Specific dates and times for the service shall be coordinated with LVAMC Biomedical Engineering staff at least 48 hours prior to initiation. Work is intended to be coordinated to minimize impact to patient treatment. Services shall be scheduled during normal business hours, between 8:00 a.m. and 4:30 p.m., Monday through Friday, excluding holidays General Responsibilities: Contractor shall notify Biomed Department immediately of any equipment failures that occur as a result of the deinstallation, relocation, or reinstallation service. Written field service reports shall be provided to a representative of Biomedical Engineering within 48 hours after completion of any equipment relocation requirements. Failure to provide Biomedical Engineering with the field service report will result in non-payment of invoices submitted for service. LVAMC will provide the necessary utilities required for connection of the systems in the new space. Upon completion of the equipment reinstallation, Contractor shall demonstrate to Biomed the full operational functionality of the equipment. Qualifications: All work shall be completed by fully qualified, field service representatives and technicians. Fully qualified is based upon training and experience and is defined as follows: Training - Field service representatives and technicians shall have completed a formalized training program within the last two (2) years and be authorized by the Original Equipment Manufacturer (OEM) to perform repair services on equipment covered under this agreement. Field service representatives and technicians shall complete refresher training on an annual basis. Experience - Field service representatives and technicians shall have a minimum of two (2) years of experience providing preventive maintenance, remedial repairs, and emergency repairs on the equipment covered in this agreement. Factory certified training certificates and competencies for all field service representatives and technicians assigned to complete the service shall be provided in accordance with Joint Commission Standards. Certificates and competencies shall be submitted to Biomedical Engineering within 14 days from date of contract award. Failure to provide required documentation will result in non-payment of invoices for service. Subcontractors, third parties, or alternate distributors shall not be used in the performance of this service Period of Performance: Contractor shall contact Biomed within 14 days of contract award to begin the coordination of the service. All work shall be completed no later than August 1, 2018.
