- Combined Synopsis for ease of viewing Attachment 3 Stadium map location data Attachment 2 connector panels Attachment 1 Cable runs PWS for Stadium Cabling Requirement Combined Synopsis/Solicitation IAW FAR 12.603 This is a combined synopsis/solicitation for a commercial service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is FA7000-18-Q-0005 and is being issued as a request for quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-97, effective 24 Jan 2018. The North American Industry Classification System Code (NAICS) is 238210 and the business size standard of $15 Million in revenue. This acquisition is a 100% Small Business Set-Aside. The following commercial service is requested in this RFQ: CLIN 0001 $__________________ (Round to the nearest dollar), QTY 1 each CLIN Description: Install, test, and label audio, fiber, coax, triax and SMPTE fiber cables and connection panels at various locations within United States Air Force Academy, Falcon Stadium to be used for TV cable patching in accordance with Performance Work Statement (PWS) and attachments. Performance shall start no earlier than 4 June 2018 and performance completion date is no later than 10 August 2018. Provision 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition Addenda to FAR 52.212-1, Instructions to Offerors: a. The contractor shall submit their quote on company letterhead to include the following: solicitation number, contact name, address, telephone number of the offeror, unit price, extended price, any discount terms, delivery terms, cage code, DUNS number, technical (descriptive/specification literature), and standard warranty information. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. Descriptive/Specification literature must contain adequate information to judge if services meet the requirements outlined in the Performance Work Statement. b. To assure timely and equitable evaluation of the proposal, the offeror must follow the instructions contained herein. Offerors are required to meet all solicitation requirements, including terms and conditions, representations and certifications, and technical requirements. Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. The response shall consist of Part I Price and Technical, Part II- Contract Documentation. c. Specific Instructions: 1. PART I - PRICE and TECHNICAL QUOTE - Submit on company letterhead the unit price for each line item to include extended price with an overall total price. All dollar amounts SHALL be ROUNDED TO THE NEAREST DOLLAR. In the technical portion, the contactor shall provide enough descriptive/specification literature to identify that they can meet the delivery and specification requirements of the Performance Work Statement and its attachments. 2. PART II - CONTRACT DOCUMENTATION (Representation and Certification). a. The provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (Mar 2015) and its Alternate I (Alt I) (Oct 2014) must be completed. Offerors are highly encouraged to complete all representations and certifications electronically online at: (https://www.sam.gov/portal/SAM/#1). If not completed online, 52.212-3 Alt I shall be completed in hard copy and submitted with quote. Subject provisions are available at http://farsite.hill.af.mil. You may contact the POC listed below for hard copies if you do not have access to Far site above. b. Price quotes must be Firm-Fixed Price. c. The System for Award Management (SAM), contractors must be registered with SAM to conduct business with the Department of Defense. No purchase order can be awarded to any company without this registration ( www.sam.gov ). d. Invoicing and payments will be completed electronically at https://wawf.eb.mil/. Contractors should register, if not already. Provision 52.212-2, Evaluation-Commercial Items, applies to this acquisition a. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Quotes will be evaluated on a lowest priced technically acceptable basis. To be technically acceptable means the contractor can meet the delivery and specification requirements of the Performance Work Statement as identified in the vendor's quote, Part 1. The government may award without discussions, however, the Government reserves the right to conduct discussions if deemed in the Government's best interest. The following factors shall be used to evaluate offers: 0 Technical Price b. The Government reserves the right to award no contract at all depending upon the quality of quotes received and whether proposed prices are fair and reasonable. Price quotes will be evaluated to determine which vendor's quote offers the lowest price to the Government if the supplies/services listed therein are purchased at the rates proposed, and to determine if prices are otherwise fair and reasonable. (End of clause) 5352.242-9000 CONTRACTOR ACCESS TO AIR FORCE INSTALLATIONS (NOV 2012) (a) The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation. (b) The contractor shall submit a written request on company letterhead to the contracting officer listing the following: contract number, location of work site, start and stop dates, and names of employees and subcontractor employees needing access to the base. The letter will also specify the individual(s) authorized to sign for a request for base identification credentials or vehicle passes. The contracting officer will endorse the request and forward it to the issuing base pass and registration office or Security Forces for processing. When reporting to the registration office, the authorized contractor individual(s) should provide a valid driver's license, current vehicle registration, valid vehicle insurance certificate, and [ insert any additional requirements to comply with local security procedures ] to obtain a vehicle pass. (c) During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site. (d) When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with [ insert any additional requirements to comply with AFI 31-101, Integrated Defense, and AFI 31-501, Personnel Security Program Management ] citing the appropriate paragraphs as applicable. (e) Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office.
