Description
This is a Sources Sought RFI. The 10th Medical Group (10MDG) at the US Air Force Academy, Colorado Springs, CO requires a one-year preventative maintenance and repair service agreement for scheduled and unscheduled maintenance on surgical arthroscopic and endoscopic equipment. Damaged and malfunctioning equipment leads to cancellation of scheduled procedures, creates a case backlog, costs the Government lost supplies and man hours, and inconveniences patients. The period of performance is approximately 01 Jul 2018 - 30 Jun 2019. Actual dates TBD. The surgical endoscopy equipment at the USAF Academy clinic was manufactured by Stryker Corporation, San Jose, CA and requires scheduled preventive maintenance and unscheduled emergency repairs to ensure equipment reliability and accuracy for surgical procedures. Proper equipment function is critical in order to enhance patient safety and surgical outcomes. This service is required for approximately one year until the operating rooms at the USAFA clinic are closed for renovation in July 2019. The service shall be performed by manufacturer trained and authorized/certified personnel using manufacturer certified components. Repairs must be completed so that any existing warranties are not voided. Repairs services must not negatively impact future warranties for parts and service going forward. The service agreement shall provide for on and off-site scheduled and unscheduled maintenance to include inspection, calibration, software updates, covered parts, and shipping costs for covered parts; unlimited repairs; loaner equipment for all covered items as required; telephone support; and travel and labor costs for a certified technician to perform required work on site. The following equipment is to be covered under this service agreement: - Visionpro 26" Led Display - L9000 Light Source - Stryker SDC3 System - Pneumo Sure Xl High Flow Insufflator - Precision HD 30 Deg., Arthroscope, 2.7mm X 75mm, C-Mount, Speedlock, A/C - Visionpro Synk 26" Wireless LED Display - Synk Wireless Transmitter - SDP1000 - 1588 Aim Camera Head - Precision HD 30 Deg., Arthroscope, 4.0mm X 140mm, C-Mount, Speedlock, A/C - Precision HD 70 Deg., Arthroscope, 4.0mm X 140mm,C-Mount, Speedlock, A/C - Precision Ideal Eyes 10mm 0°, HD Autoclavable Laparoscope 33cm - Precision Ideal Eyes 10mm 30°, HD Autoclavable Laparoscope 33cm - Precision Ideal Eyes 5.5mm 0°, HD Autoclavable Laparoscope 30cm - Precision Ideal Eyes 5.5mm 30°, HD Autoclavable Laparoscope 30cm Delivery is FOB Destination to USAF Academy, CO 80840.
