- CONTRACT TITLE: Pak Plus System Robot RX overwrap, bulk and single-dose packaging and barcoding of medications for Pak Plus systems. BASE YEAR: OCT 1, 2018 THRU SEPT 30, 2019 + four option years. BACKGROUND: The Department of Veterans Affairs; Puget Sound Health Care System, Seattle Division requires onsite services for over-wrap, bulk dose, and single dose packaging of medications. The contractor shall furnish all labor, equipment and consumable materials to perform unit dose barcode packaging in preparation for use of the VA owned McKesson Robot-RX Automated Medication Dispensing System; and single dose packaging that does not contain a barcode as specified in the statement of work. SCOPE: A contractor will use the Pak Plus system to provide medication packaging services at the Seattle Division, VA Puget Sound, 1660 S. Columbian Way; Seattle, Washington, 98108. SPECIFIC TASKS/PLACE OF PERFORMANCEE/PERIOD OF PERFORMANCE: The contractor shall provide at a minimum one full time primary and one alternate technician for VA Puget Sound, Seattle Division to perform on-site pharmaceutical packaging services utilizing packaging equipment owned by the contractor. Packaging shall be compatible with the VA owned automated high-speed Robot RX Ready Medication Dispensing Distribution System. Technicians will also be required to perform Non-Robot-RX Ready packing for single-use dosage forms that do not contain a manufacturer s barcode. Services shall be performed at the VA Puget Sound Health Care System Seattle, Washington 98108. The contractor shall perform work between the hours of 08:00AM 5:00PM Monday Friday Excluding Federal Holidays except by special arrangements with the facility s pharmacy manager. The contract shall be for one (1) base year plus 4 option years. SECURITY REQUIREMENTS: All contractor employees are subject to the same level of investigation as VA employees who have access to VA sensitive information or access to VA facilities. The background investigation includes the following requirements: 1) Completed documentation 2) Fingerprints 3) Completion of OPM s e-QIP Questionnaire. The Contractor is required to fulfill all of the security requirements. The Contractor, upon completion of fingerprinting, and an initial suitability determination, may be authorized tentative access to start the performance period of the contract, but only on condition of completion of all security requirements. This requirement is applicable to all subcontractor personnel requiring the same access. The contractor shall bear the expense of obtaining background investigations. If the investigation is conducted by the Office of Personnel Management (OPM) through the VA, the contractor shall reimburse the VA within 30 days. The contractor employee shall be required to complete security training and sign a Department of Veterans Affairs (VA) computer Access Agreement prior to having access to VA computer system. In addition, if providing medical services, contractor employee will attend computerized patient record system (CPRS) training prior to providing any patient care services, including on-call and emergency coverage at VA. The contractor employee will document patient care in CPRS to comply with all VA and JCAHO standards. As the VA routinely reviews and updates policies and procedures covering contractor computer access, this contract may be modified to reflect new policies and procedures implemented during the term of this agreement PERSONNEL QUALIFICATIONS: The contractor shall ensure all personnel performing services under this contract are qualified. Qualified for the purpose of this contract means each technician is trained and capable of performing both over-wrap and bulk packaging medications through the use of Pak-Plus Rx for the Robot Rx automated medication dispensing distribution system, and non-Robot-RX ready packing for single use dosage forms that do not contain a manufacturer s barcode. The contractor shall have qualified personnel available and ready to perform at both facilities (VAPSHCS) within 30 days of contract award. Contractor-hired personnel performing services under this contract shall be governed by an agreement between the contractor and the hired service personnel. All supervision and performance monitoring of hired personnel shall be the responsibility of the contractor. PERFORMANCE MONITORING: Performance objective shall be defined as the successful completion of all on-site Pak-Plus services as described within the statement of work. The daily performance summary log and packaging log shall be used as evidence of successful completion. CONTRACTOR S RESPONSIBILITIES: The contractor shall: Furnish and provide in-house (Robot and Non-Robot) Pak-Plus RX ready medication packaging services to include over wrapping, unit packaging with barcode, and single-use dosage packaging that do not contain a manufacturer s barcode. Over-wrap each dosage forms that are tablets or capsules with the United States Pharmacopoeia (USP) standard material so that the dose is Robot-RX ready and label the outer wrap with the name and strength of the drug, lot number and expiration date. Ensure bar codes are legible. No more than one reject per 10,000 does is acceptable. Comply with procedures, policies and guidelines established by the contractor s manager and / or the VA pharmacy service representative. Monitor and manage off-line inventory levels to ensure availability of inventory for picking by Robot-RX. Print a daily repackaging report identifying drugs/medications required to be packaged to ensure that robot RX off-line inventories are at optimal levels. Ensure 100% of all routinely used medications are available at all times. This includes medications routinely dispensed using the Robot-RX ready for over wrap and bulk packaging and Non-Robot Rx-ready packing for single use dosage forms that do not contain a manufacturer s barcode. Have a mechanism for incorporating new products into inventory with two (2) days of identification of need for the new item. Coordinate with VAPSHCS pharmacy technicians in order to maintain sufficient product inventory and to ensure prompt processing of medication. Maintain an inventory and submit invoices for products checked for accuracy by VA staff and approved by the station pharmacist. The contractor shall provide, at the end of each shift, one (1) copy of the daily performance summary log to each respective medical center s (VAPSHCS) pharmacy COR. The VA reserves the right to periodically perform audits on any performance documented on the summary log. Update the Robot RX inventory reports by inputting relevant packaging information in the computer database to include quantities. Provide, order, and pay for supplies needed for the operation of the packager (i.e. bags, foil, tape, etc. including monitoring of the inventory of these supplies. Submit a report documenting the weekly usage to the VA pharmacy service COR. Consult and meet with pharmacy staff regarding means of improving general packaging services. Provide the VAPSHCS with a primary point of contact for operational issues related to packaging. The contractor shall maintain its own packaging equipment used during the performance of this contract. GOVERNMENT RESPONSIBILITIES: The VA Medical Center shall: Allocate a primary pharmacy service contact to serve as the responsible party in to work with the contractor project manager and provide direction to the contractor s on-site technician(s) during performance this contract. Enter and validate prescription orders into the pharmacy information system for Robot- RX medication dispensing. Provide ADT and cart-filled data via an on-line interface to the Robot-RX medication dispensing system. Perform activities that must be performed by a pharmacist under applicable law. Appoint a COR responsible for certifying that the work done under the contract is performed to time and standard. SAFETY AND FIRE PREVENTION: The contractor shall provide all safety equipment for their employees in the performance of this contract while on Government property and shall abide by safe working standards. During the performance of this contract, the contractor shall take such safety precautions as necessary to protect the lives and health of occupants of the buildings. The contractor shall immediately correct any fire and safety deficiencies caused by his/her employee. If the contractor fails or refuses to correct deficiencies promptly, the contracting officer may issue an order stopping all or any part of the work and hold the contractor in default of the contract. The contractor shall comply with applicable Federal, State, and Local safety and fire regulations and codes, which are in effect at the beginning of the contract period. The contractor shall keep abreast of and comply with changes in these regulations and codes applicable to the contract. The contractor is to retain copies of all MSDS forms for all chemical products used, and EPA must approve all chemicals used. All contractors, its supervisors and employees performing services Government installation shall comply with the Hospital and Safety regulations, Occupational Safety and Health Act (OSHA) PRIVACY OFFICER LANGUAGE FOR STATEMENT OF WORK (SOW) Information made available to the contractor or subcontractor by VA for the performance or administration of this contract or information developed by the contractor/subcontractor in performance or administration of the contract shall be used only for those purposes and shall not be used in any other way without the prior written agreement of the VA. This clause expressly limits the contractor/subcontractor's rights to use data as described in Rights in Data- General, FAR 52.227-14(d) (1). VA information should not be co-mingled, if possible, with any other data on the contractors/subcontractor s information systems or media storage systems in order to ensure VA requirements related to data protection and media sanitization can be met. If co-mingling must be allowed to meet the requirements of the business need, the contractor must ensure that VA s information is returned to the VA or destroyed in accordance with VA s sanitization requirements. VA reserves the right to conduct on-site inspections of contractor and subcontractor IT resources to ensure data security controls, separation of data and job duties, and destruction/media sanitization procedures are in compliance with VA directive requirements. Prior to termination or completion of this contract, contractor/subcontractor must not destroy information received from VA, or gathered/created by the contractor in the course of performing this contract without prior written approval by the VA. Any data destruction done on behalf of VA by a contractor/subcontractor must be done in accordance with National Archives and Records Administration (NARA) requirements as outlined in VA Directive 6300, Records and Information Management and its Handbook 6300.1 Records Management Procedures, applicable VA Records Control Schedules, and VA Handbook 6500.1, Electronic Media Sanitization. Self-certification by the contractor that the data destruction requirements above have been met must be sent to the VA Contracting Officer within 30 days of termination of the contract. The contractor/subcontractor must receive, gather, store, back up, maintain, use, disclose and dispose of VA information only in compliance with the terms of the contract and applicable Federal and VA information confidentiality and security laws, regulations and policies. If Federal or VA information confidentiality and security laws, regulations and policies become applicable to the VA information or information systems after execution of the contract, or if NIST issues or updates applicable FIPS or Special Publications (SP) after execution of this contract, the parties agree to negotiate in good faith to implement the information confidentiality and security laws, regulations and policies in this contract. The contractor/subcontractor shall not make copies of VA information except as authorized and necessary to perform the terms of the agreement or to preserve electronic information stored on contractor/subcontractor electronic storage media for restoration in case any electronic equipment or data used by the contractor/subcontractor needs to be restored to an operating state. If copies are made for restoration purposes, after the restoration is complete, the copies must be appropriately destroyed. RECORDS MANAGEMENT LANGUAGE FOR CONTRACTS FOR SOW Citations to pertinent laws, codes and regulations such as 44 U.S.C chapters 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. The Government Agency owns the rights to all data/records produced as part of this contract. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.]. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under, or relating to, this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information.
