- TEST SET, INSTRUMENT No telephone requests will be accepted. Direct your requests and communication to: ATTN: DLA-AHA, Hiram Banuchi, Contract Specialist, via email hiram.banuchi@dla.mil or facsimile 256-955-2460. For request for solicitation, provide your name, the company name and Cage Code and a telephone/facsimile number. PRON: AX8F1106AX. The Government proposes to acquire, on a Firm-fixed Price contract, a quantity of 18 each min / 90 each max of a TEST SET, INSTRUMENT; NSN: 4920-01-592-6411; Part Number: 06-0125-65 applicable to the BE UH-60 (Blackhawk) weapon system. Forecasted/estimated quantities: Year 1: 18 each Year 2: 18 each Year 3: 18 each Year 4: 18 each Year 5: 18 each Production Lead Time without First Article Testing is 18 each, 180 Days After Contract Award (DACA) with deliveries to TEXARKANA, TX 75507-5000. This item is restricted to ULTRAX AEROSPACE INC., CAGE CODE: 07PR7. The Government does not have adequate data, lacks rights to data, or both needed to purchase this part from additional sources. This must be acquired from a manufacturing source(s) specified on a source control drawing. All offerors must meet prequalification requirements in order to be eligible for award. Firms that recognize and can produce the required item described above are encouraged to identify themselves. Firms are encouraged to seek source approval in order to compete for future solicitations by accessing the website: http://amcomdmz.redstone.army.mil/casl_cmo/casldba.casl_cmo_samsar and looking under Standardized Aviation and Missile Source Approval Request (SAMSAR). All responsible sources may submit an offer, which shall be considered by the Agency. The closing date annotated is an estimated date and may be adjusted dependent upon the date of release for solicitation; however, the solicitation will not close prior to the date stated above. Foreign Firms are reminded that all requests for solicitation must be processed through their respective embassies. THIS ACQUISITION CONTAINS TECHNICAL DATA WHOSE EXPORT IS RESTRICTED BY THE ARMS EXPORT CONTROL ACT (TITLE 22, USC., SEC 2751 ET SEQ) OR THE EXPORT ADMINISTRATION ACT (TITLE 50, USC., APP 2401-2402) EXECUTIVE ORDER 12470.
