- This sources sought notice is for information and planning purposes only, and shall not be construed as a solicitation or as an obligation on the part of the Department of Veterans Affairs (VA). The VA is currently conducting market research to locate qualified, experienced and interested potential sources relative to NAICS 624229. Responses will not be considered as proposals, nor will any award be incurred by interested parties in responding to this sources sought announcement. In accordance with FAR Part 10, Market Research, this is a sources sought to determine the availability of potential sources/vendors having the skills and capabilities necessary to provide all personnel and facilities necessary for providing Safe Haven Services to the Northern Indiana Health Care System (NIHCS) Catchment Area. All interested contractors are invited to provide information to contribute to this market survey/sources sought including, commercial market information. General Requirement The VA has been providing direct and specialized services for homeless Veterans for over 25 years. As part of the Plan to End Homelessness among Veterans announced in late 2009, the Veterans Health Administration (VHA) is funding programs and services to homeless Veterans. Recent national trends in service provision for the homeless outside of the VA system include the utilization of Safe Havens as an alternative to shelter care or as a placement after unsuccessful discharge from an inpatient or residential treatment program due to relapse-related issues. A Safe Haven is a form of treatment housing that serves hard-to-reach homeless persons with severe mental illnesses and/or substance abuse disorders that are on the streets and have been unable or unwilling to participate in more traditional treatment services. It is a low demand model of service delivery that allows clients to engage in treatment and services at their own pace. (For an explanation of Safe Havens, please view the following web sites: https://www.onecpd.info/resources/documents/SafeHavens.pdf and http://www.wardfamilyfoundation.org/wff-safehaven-programs.pdf The intent of this contract is for NIHCS to engage a community provider to offer homeless Veterans services through a Safe Haven model of residential care. The NIHCS will solicit community organizations to provide Safe Haven services as a specialty model under the contract authority of VA s Healthcare for Homeless Veterans (HCHV) Contract Residential Care programs, (For an explanation of the HCHV Contract Residential Program, view the following web site: http://www.va.gov/homeless/hchv.asp The Northern Indiana Health Care System (NIHCS) is using this model of HCHV Contract Residential Care, herein after referred to as a Safe Haven, to offer services for twenty-five (25, estimated quantity) chronically street homeless Veterans under the HCHV Contract Residential Care program enhanced by the services as defined in the Tasks and Deliverables. Significant deviation from these services can dilute the model as prescribed, and thus disrupt program evaluation and fidelity studies. The VA National Center on Homelessness among Veterans ( the Center ) is available for consultation for program development of both providers and VA personnel. Additionally, the Center has formulated guidelines, protocols, and fidelity measures which must be adhered to by the selected Contractor and the NIHCS. Many of the veterans involved in this program have co-morbid conditions that include serious mental illnesses, substance abuse disorders, and other serious medical conditions. The goal of the HCHV Contract Residential Care Safe Haven Program is to rapidly stabilize Veterans medical, mental health, substance abuse and other psychosocial problems in order to place Veterans in other appropriate transitional or permanent housing within a maximum of 180 days. NIHCS intends to engage a Contractor to provide rapid placement of VA-referred Veterans in a safe, residential treatment setting while addressing Veterans complex needs through on-site treatment case management services using a Safe Haven model. This housing must meet the criteria established both by HCHV clinical staff and VA fire and safety officials. Requested submittal Information: Contractors having the skills and capabilities necessary to perform the stated requirements should submit a response of no more than 10 pages in length, single spaced, 12 point font minimum. The Government will not review any other data or attachments that are in excess of the 10 pages. Companies should provide clear and unambiguous evidence to substantiate their capability to fulfill these requirements. Interested parties shall furnish the following minimum information: 1. Company name and address. 2. Company point of contact name, telephone number and email address. 3. Is your business large or small? 4. If small, does your firm qualify as a small, emerging business, or small disadvantaged business? 5. If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act 6. Is your firm a certified Hub zone firm? 7. Is your firm a woman-owned or operated business? 8. Is your firm a certified Service-Disabled Veteran or Veteran Owned Small business (SDVOSB or VOSB)? 9. If your firm is a SDVOSB or VOSB, are you registered with VetBiz Registry? If not, please register at: http://vip.vetbiz.gov/general_user/register/default.asp 10. Does your firm have a DUNS number? If so, please provide. If not, you must register with Dun & Bradstreet at: http://www.dnb.com 11. Capabilities/Qualifications: A written response providing clear and unambiguous evidence to substantiate the capacity to fulfill this requirement. Description of the capabilities/qualifications/skills your company possesses to perform services described in the scope of work. 15. Do you need to subcontract any portion of work to provide these services stated herein? 16. Past Experience: Brief summary of the company history relative to similar requirements. Offer may provide the following information on a maximum of three similar projects for which the responder was a prime or subcontractor. a. The name, address, and value of each project. b. The Prime Contract Type, Firm Fixed-Price, or Time and Material. c. The name, telephone and address of the owner of each project. d. A description of each project and why it is relevant to this requirement. Include difficulties and successes. e. Your company s role and services provided for each project. 17.
