- statement of work, photos, and floor plan The contractor shall provide all personnel, equipment, tools, materials, and supervision necessary to replace/repair/service asbestos. ACM to HEPA VAC areas, glove and bag and encapsulate the areas, clean area upon completion of project. Please submit all proposals along with contractor past performance references should be sent via email: Sherida Hardy; sherida.hardy@dm.usda.gov •Secure contaminated area to prep for asbestos removal. •Provide barrier to protect the area of work during work. •Provide protection for the items that remain in space during work. •Remove the existing suspended ceiling tile system in the office area. •Remove cork panels where concrete repair work is required. Encapsulate edges of the cork panels to remain. •Remove cork panels that have fallen or detached from ceiling; remove loose cork panels; encapsulate edges of panels to remain. •Remove cork debris on ceiling tiles. •Removal work and disposal shall follow federal and local regulations. •Submit waste manifest within 30 days after the waste is removed from the building. •Provide air monitoring, testing, reporting, and final clearance tests. If failed, contractor shall repeat the abatement work until the final clearance tests pass •Dispose of the waste as directed according to Federal Laws. •Clean work area at all times and upon completion. •Contractor shall provide operational checks and balance of the system upon completion. •All work to be done during normal and off hours. •The Contractor shall repair or replace any damage to the existing conditions caused by his actions. Repair and replacement of damaged conditions shall match the integrity of the adjacent area. •Provide USDA with manifest of abated products and manifest of disposal. •Contractor shall ensure final project meets current Guidelines, FAR regulations and current appropriate codes, standards, and regulations relative the United States Department of Agriculture. •Contractor shall immediately notify USDA safety personnel of all accidents/incidents involving employee use of and/or damage/injury to facilities, equipment, or personnel. •Contractor shall adhere to all OSHA standards. •Contractor shall comply with all federal, state, and local environmental laws, and USDA policies and regulations to include but not limited to Resource Conservation and Recovery Act (RCRA), Safe Water Drinking Act {SWDA), the Clean Air Act (CAA), and Federal Facilities Compliance Act (FFCA). •The use, handling, storage, and disposal of all toxic, hazardous, special or radiological wastes/materials shall be IAW all local, state, federal, and USDA environmental regulations and procedures. Work to be performed at: US Department of Agriculture 1400 Independence Avenue SW Washington, DC 20250 All proposals along with contractor past performance references should be submitted by 5:00pm (eastern) Monday, June 11, 2018.
