VA NEW YORK HARBOR HEALTHCARE SYSTEM MENTAL HEALTH SERVICE 423 East, 23rd St New York, NY 10010 STATEMENT OF WORK FOR COMMUNICATION EQUIPMENT/TECHNICAL SUPPORT FOR THE TELE-MENTAL HEALTH HUB 2237#: 630-17-3-4924-0101 Introduction/Background The VA New York Harbor Healthcare System is dedicated to providing quality health care to veterans using the abilities of all employees supported by our commitment to education and research. We are recognized locally, regionally, and nationally as a leader in quality patient care, positive customer service, medical/allied health education, and health-related research and employment opportunities. We seek to be the provider of choice of veterans and the community by offering an efficient, integrated quality health care system capable of providing a full range of primary, specialty and chronic health care services in a system that is readily accessible and responsive to change. Our key business drivers are quality, financial integrity, patient satisfaction and employee satisfaction. The Mental Health Service, at the New York Harbor Healthcare System provides general and specialty mental health services for treatment of the broad range of mental health conditions, including treatment for substance abuse disorders, post-traumatic stress disorder, military sexual trauma (discussed in detail below), and serious mental illness. Services are provided in the outpatient and inpatient mental health setting. General and psychogeriatric mental health services are provided as an integral part of primary care within the Patient Aligned Care Teams (PACT) as well as VA nursing homes and residential care facilities where many Veterans receive care. Objectives/Requirements What: Mental Health needs to purchase the following equipment with technical support coverage. Jabra BIZ 2400 II Headset - Mono - USB - Wired - Gold Plated - Over-the-head - Monaural - Supra-aural - Noise Cancelling Microphone - Yes QD Mfr Part # : 280-09 JABRA LINK 280 USB CORD - GN QD INLINE SW UPGRADABLE CONTROLS Service Level Agreement FEE Participants: Veterans from various VAMCs: Bath and Iowa. When: Ongoing treatment via scheduled appointments. Where: Via CVT between veterans and Mental Health clinicians at the NY Harbor. Justification Clinical Video Telehealth means that instead of having the cost and inconvenience of traveling by road, rail or air to see a specialist in the hospital the specialist comes to you. If Veteran's need medication prescriptions changed or if the veteran needs to see the specialist in person, have special investigations or come into hospital then this can even be arranged through Clinical Video Telehealth. Purpose of treatment Severe depression. Long-term depression that does not improve with medication or other treatments. Schizophrenia when symptoms include psychosis, suicidal ideation, a desire to harm someone else or refusal to eat. Bipolar disorder when symptoms include severe mania. Signs of mania include impaired decision making, impulsive or risky behaviors, substance abuse and psychosis. Schizophrenia and some other psychiatric disorders where catatonia is a symptom. Catatonia is characterized by lack of movement or strange movements and difficulty with speech. Product Delivery/Technical Support The vendor shall be responsible for determination of and compliance with federal and state or local code requirements, design data, and other factors necessary to deliver the items at the desired location. (a) The price quoted shall include contractor responsibility for providing on-site delivery and technical Support for Cisco DX80. Place of Delivery The product will be delivered to Mental Health Service Attn: Mary Hinkson 423 East 23rd St. 17th Floor West New York, NY 10010 Time Of Delivery Services will need to be provided by July 18, 2018. Product Installation/Assembly The vendor shall be responsible for determination of and compliance with federal and state or local code requirements, design data, and other factors necessary to install or deliver the items at the desired location. The system will not require any new construction, space, special wirings etc. (a) The price quoted shall include contractor responsibility for providing on-site delivery, assembly, orientation and training of using personnel in operation and care of the equipment furnished. This training shall include actual demonstration and operation of the equipment, preventative maintenance, and any adjustments or other actions which may be undertaken by operating personnel in the event of failure of equipment, provided that such adjustment or action shall in no way jeopardize the Government's rights under contract guarantee clause. Upon completion of assembly/installation, this training shall be given by qualified contractor representatives on a date to be determined by the using service POC. Operator and maintenance training shall not be conducted concurrently. (b) Vendor shall submit training materials and training videos (prepared in CD or DVD format) on each piece of equipment for operation and maintenance. Any available training videos shall be for operation and maintenance and shall not be combined. These training materials and videos shall demonstrate the operation and maintenance of the equipment and any adjustments or other actions that may be undertaken by VA operating personnel in the event of failure of equipment. These training materials and videos shall be submitted to the responsible operations official or his designee prior to beginning of training. (c) The Vendor shall consult with the Contracting Officer Representative or person acting in that capacity regarding the time this training will begin. These officials will be responsible for arranging for the presence of personnel to be trained. Important Note: Information Security and Privacy Training All contractor employees and subcontractors participating under this contract or order, to provide installation, training or service repairs are required to complete the VA's Privacy and Information security training requirement VA Privacy Training for Personnel without Access to VA Computer Systems  or Direct Access or Use to VA Sensitive Information Contractors must provide signed certifications of completion to the COR during each year of the contract.   This requirement is in addition to any other training that may be required of the contractor and subcontractor(s). Recalls The Vendor shall immediately notify all affected VA Medical Centers and designated POC in the event of a device recall, removal, required adjustment, or required modification that is suggested or mandated by the Vendor, distributor, manufacturer, or any regulatory or government agency. The following information shall be included in this notification: A complete item description and identification (to include lot and serial numbers where applicable); Administrative identification data including order numbers and order date; Reasons for recall, removal, adjustment, or modification; and Instructions for appropriate corrective action.
