- Specifications and Scope of Work Z-PNW Forestry Science Lab HVAC Replacement in Corvallis, OR SOURCES SOUGHT ONLY ANNOUNCEMENT TO REQUEST INFORMATION FROM INTERESTED CONTRACTORS: The USDA Forest Service desires to award a single firm fixed price construction contract. We encourage all eligible vendors to identify their capabilities in meeting the requirements as a fiar market price. See 1 attachement labeled Specs_Scope of Work for all project details. Estimated magnitude of construction of between $5,000,000 and $10,000,000 Contractors must be able to secure a 10% Bid Guarantee with their proposal and a 100% Performance and Payments Bond after award is made. Interested vendors shall submit responses by email to lrakich@fs.fed.us. Responses must be received no later than June 29, 2018. Oral communications are not acceptable in response to this notice. SUBMISSION REQUIREMENTS 1. Offeror name, address, point of contact, phone number and email address. 2. Firms DUNS number. 3.Average annual revenues over each of the previous 3 years. 4. # of currents employees represented in each trade discipline (not subcontractors or 1099 personnel) 5. Project Manager personnel qualifications 6. In house A&E and professional engineer capabilities 7. Bonding Capacity (construction bonding level per contract and aggregate construction bonding level, both expressed in dollars). 8. Brief description of up to 10 similar projects completed by your company as a prime contractor within the last 5 years, including as much of the following information as possible: (a) total dollar value of the contract, (b) brief description of the technical requiremtns of the project, (c) a description of the work that was performed and whether it was totally self-performed or what portions were subcontracted, (d) performance period, (e) name, position, phone number, and email for a customer representative: Experience should be demonstrated in the following areas: 1. Repair, replacement, and maintenance of commercial-grade mechanical, electrical and plumbing systems. 2. Interior remodeling work including rough and finish carpentry, painting, floor and wall covering. 3. Exterior remodeling work, including rough and finish carpentry, painting, repair or replacement of roofing, siding, windows, doors or other miscellaneous architectural elements. 4.
