Description
This is a SOURCES SOUGHT only pertaining to the supplies/services noted below for the Indianapolis VA healthcare facility. Information collected during this Sources Sought may be used in a set aside. If a solicitation is issued, the Government will do so in accordance with Federal Acquisition Circular (FAC) 2005-83. The North American Industry Classification System (NAICS) number is 339116. The NAICS size is 500 employees. Any contractor that believes they are capable and desires to claim preference for small business status must be registered with the SBA at http://web.sba.gov/pro-net/ and meet the requirements of FAR 19.102. Any contractor that believes they are capable and desires to claim preference for veteran owned small business status must be registered with the VIP at https://www.vip.vetbiz.gov as an SDVOSB or VOSB. A Contractor that proposes to provide a supply that is not manufactured by their organization (nonmanufacturer) shall meet the requirements of FAR 19.102(f). Contractors that deem themselves capable of meeting the requirement shall provide the below information to Kellie Konopinski, Contracting Officer, at Kellie.Konopinski@va.gov no-later-than Monday, June 11, 2018 at 4:30PM, EDT, referencing (36C25018Q9242). Responses shall include: (1) Business Name and Address, (2) GSA Contract Number, if applicable (3) Point of Contact Name, Phone Number and E-mail Address (4) DUNS and NAICS code (5) Business Size SMALL or LARGE (6) Type of Small Business (if applicable): service disabled veteran owned, veteran owned small business, 8a, HUBZone, woman-owned, etc. (7) A concern that is not a manufacturer of the supply shall provide the manufacturers name and size standard. If the concern is the manufacture and the sole distributor, please provide documentation to that effect. Contractor must be registered with https://www.sam.gov To be considered SDVOSB/VOSB, must be registered in VIP (https://www.vip.vetbiz.gov/). Supplies are to include only TAA compliant items. Items that are not TAA compliant must be clearly identified by the Contractor should the contractor choose to offer a quote if a RFQ is published. FOB is Destination. Delivery is to: VA Medical Center 1481 West 10th Street Indianapolis, IN 46202 REQUIREMENT IS FOR A BASE AND 4 YEAR IDIQ CONTRACT: DENTAL LAB SERVICES 1. GENERAL: This is a non-personnel services contract to provide dental laboratory services to construct fixed and removable prostheses for eligible veteran beneficiaries. The Government will not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government. 1.1 Description of Services/Introduction: The Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform dental laboratory services as defined in this Performance Work Statement except for those items specified as Government furnished property and services. The Contractor shall perform to the standards in this contract. 1.2 Background: The Richard L. Roudebush VAMC (RLRVAMC) provides health care services to veterans of the U.S. armed services. Part of the care provided to veterans involves providing dental prosthetics. While the RLRVAMC has a robust dental service it requires outside services for dental prosthetics in order to meet the needs of the Veteran. 1.3 Objectives: Provide transportation for pickup and delivery of dental prosthetic cases. Provide all necessary dental lab supplies to construct prostheses Construct prosthesis according to provider s prescription and industry quality standards Provide timely and accurate completion of dental lab cases: The contractor s shipping/delivery times will add on only two (2) days to the contractor s published turnaround times (one day for morning pick-ups and one day for delivery). However, for cases picked-up after 1:00 p.m., the contractor may add only one (1) additional day for shipping/delivery. Pick-up and delivery shall be between the hours of 8:00 a.m. 4:00 p.m, Monday through Friday. Patient casework shall be identified with patient s last name, last name of patient s provider, and patient s next appointment date. For standard implant cases, the contractor may add one (1) day to the contractor s published turnaround times. The contractor shall meet the following turnaround times (additional turnaround times must be negotiated between the Dental Service Chief, COR, and CO in special cases): FCC- four (4) days PFM and Veneers- six (6) days Ceramics/non-metallic crowns- eight (8) days Set-ups- three (3) days Denture finish- four (4) days Set-up/finish five (5) days Frameworks- six (6) days Soft relines two (2) days Provisional crowns- four (4) days Implant crowns for cementation of UCLA design- ten (10) days Implant restorations (including custom abutments) ten (10) days Emergency repairs and/or relines twenty-four (24) hours The contract will handle complex, full-mouth implant cases when required by the RLRVAMC Consult with the dental provider as needed Make any necessary changes after the provider has reviewed the final product for quality if changes are ordered by the provider.
