Description
This is questions and answers to share with all interested vendors. Also the first CLIN 'collection kits' will be changed from 80 to 20,000.00. The purpose of this modification is to first change CLIN 1 from a quantity of 80ea to a quantity of 20,000.00 ea. Secondly some questions and answers for the benefit of all interested vendors is: Questions and Answers A: If orders are placed by fax or email" should be "A: Orders are placed by fax or email." Technical Appendix A 1.4.3 W91YTZ18Q0120 Q: Will the Government delete the assessment of the contractor's ability to deliver o or before the third business day after the order is placed as the Government's FedEx Contractor is responsible for delivery. A: No, the 3rd business day is for the **COLLECTION SUPPLIES,* not the collected urine. Requests to the vendor for collection supplies should be delivered to the destination unit within 3 business days of the vendor receiving the request if possible. Q: A quantity of 80 seems too low for CLIN 0001. Shouldn't it be more? A: The urine kit is changed to 20,000.00 ea. Question: Two NAICS's were used 621511 and 339112. Which NAICS do we need to be a small business under? Answer: The NAICS code is a commodity code that the vendor registers his/her products under. So if the requested supplies have been associated with an NAICS that the vendor is considered a small business under then either one could work. Others could potentially work as well. Additional End User Answers A:. Yes, the FGM FTDTL has a FedEx account that is used to ensure a) timely shipment and delivery of collected urine specimens and b) as required of us in contract HQ0034-13-C-0127. Because it is imperative that collected specimen arrive quickly and accurately and give the FGM the ability to reconcile what was sent with what arrives, we can't allow vendor to handle the shipment of collected specimens to us. The FGM FTDTL will provide vendor with our FedEx labels. A:. Having said that, the vendor DOES need to understand they will be shipping supplies all over the world on their dime, not the FGM FTDTL's. The FTDTL only pays for shipment of collected urine specimens to our lab. A:. Shipment of supplies do NOT come to FGM FTDTL, they go directly to the unit that has ordered the supplies. Only collected urine comes to the FGM FTDTL. A: No, the urine collection kits do not need to be sterile
