Amendment 00004 to COMBINED SYNOPSIS-SOLICIATION 36C24618Q0798 The purpose of this Amendment 00004 to COMBINED SYNOPSIS/SOLICIATION 36C24618Q0798 is to schedule site visit, define working hours, add new evaluation factors, and extend the closing date. Site visit is scheduled as follows: Date: Tuesday, June 19, 2018 Time: 10:00 AM EST Location: Police Office, Building 1, Basement 2300 Ramsey St. Fayetteville NC 28301 Purpose: To view the site for CCTV system upgrade, installation, and training. To answer technical questions. Local/Technical Point of Contact: Stephen Connor, Chief of Police No further questions will be answered by Contracting Officer (CO) until after the site visit. Chief Connors will answer any technical questions during the site visit. All questions after the site visit shall be addressed to Melisha Evans, CO, via email Melisha.Evans@va.gov. All questions or inquires must be submitted no later than 12:00Noon Eastern Standard Time on June 22, 2018. No phone calls will be accepted. Working Hours: All work is to be done during normal business hours, Monday Friday from 7:00am to 5:00pm. Weekend work requires prior coordination with Stephen Connor, Chief of Police. New Evaluation Factors added due to the technical nature of the upgrade/installation: 52.212-2 Evaluation-Commercial Items: The Government intends to award a firm-fixed-priced contract resulting from this solicitation to the responsible vendor whose quote conforming to the requirements in this request for quote will be most advantageous to the government, price and other factors considered. The following factors (in order shown) shall be used to evaluate offers: Lowest firm-fixed price that meets the factors below. Quote considered or not considered based on whether proposed service meets the requirements of the SOW. Past Performance: Present and Past Performance shall be evaluated as a measure of the Government s confidence in the offeror s ability to successfully perform based on previous and current contract efforts. Past Performance rating of acceptable or non-acceptable will be determined by the technical team at the Fayetteville VAMC. Vendor will be considered for award only after meeting factors 1 and 2 above and meeting all other requirements of IV VIII above. If an offer is not chosen after the above factors are evaluated, the cycle begins with the next lowest firm-fixed price until an offer is accepted. If all offers are unacceptable, this solicitation will be cancelled. Closing date for quote response has been extended. Quotes are due by 04:00PM Eastern Standard time on June 29, 2018. Quotes received after this date and time will be considered late in accordance with 52.212-1(f) and will not be evaluated. Written quotes shall be submitted via email to Melisha.Evans@va.gov. NO FAXED OR MAILED OFFERS WILL BE ACCEPTED. All submissions become part of the procurement file. No other changes were made.
