Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation number WESU8089 is being issued as request for quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-96. (iv) This requirement is being issued in accordance with 25 USC 47, Buy Indian Act, with preference to be given to Indian Small Business Economic Enterprises (ISBEE). Other small businesses are encouraged to submit an offeror as they will be an opportunity. The associated NAICS code is 6213299 and small business size standard is $7.0 Million. The pricing schedule shall be structured per the following: 0001: 1 base year services 0002: 1st option period 0003: 2nd option period 0004: 3rd option period 0005: 4th option period See the attachment B and SF - 1449. (vi) This requirement is to support David C. Wynecoop Memorial Clinic, 6203 Agency Loop Road, Wellpinit, Washington 99040. (vii) This requirement includes 1 base year period and 4 one year option periods. The Delivery and Acceptance terms for the contract are FOB Destination. (viii) The provision at 52.212-1, Instructions to Offerors - Commercial (Jan 2017), applies to this acquisition. Following Addendum applies to this RFQ: The submission shall include the following information - 1)Company name 2)POC 3)PH Number 4)DUNS etc. (ix) The provision at 52.212-2, Evaluation -- Commercial Items, 52.212-2 -- Evaluation -- Commercial Items (Oct 2014) applies to this RFQ. (a)The Government will award a firm fixed price contract to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The Government reserves the right to award more than one contract from this solicitation based on best value. (b)The following factors shall be used to evaluate offers for submitted RFQs. a)Technical Capability b) Key Personnel c) Relevant Past performance d) Price The technical factors, when combined, are more important than price. Please see the attached evaluation factors/quotation submission instruction for more information. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
