This is a Sources Sought Notice (a) The Government does not intend to award a contract on the basis of this Sources Sought or to otherwise pay for the information solicited. (b) Although "proposal," "offeror," contractor, and "offeror" may be used in this sources sought notice, any response will be treated as information only. It shall not be used as a proposal. (c) Any information received from a contractor in response to this Sources Sought may be used in creating a solicitation. Any information received which is marked with a statement, such as "proprietary" or "confidential," intended to restrict distribution will not be distributed outside of the Government, except as required by law. (d) This Sources Sought is issued by VISN 2 Contracting Office in Albany, NY for the purpose of collecting information about procurement motorized parallel bars at the Northport VAMC at Northport NY. The VA is seeking to provide for this requirement as of July 7th 2018. The NAICS code identified for this requirement is 339920 Sporting and Athletic Goods. The requirement is detailed in the Statement of Work section of this document. (e) Cursory market research has identified several small businesses that may be able to provide for this requirement. Contractors that are able to meet the requirements in the attachment are encouraged to email a capability statement and full information to Dayna Dickson at Dayna. Dickson@va.gov - Part of the purpose of this sources sought is to determine the viability of set aside to a specific socio-economic category. Contractors shall identify the NAICS code for the services being offered as well as their size status under said NAICS. If contractor is a Service Disabled Veteran Owned Small Business (SDVOSB), a Veteran Owned Small Business(VOSB), or any other special category of contractors, then contractor is responsible for providing documentation in support of this. STATEMENT OF WORK (SOW) 632-18-3-3086-0243 Northport VA Medical Center, Northport NY (632) Physical Medicine and Rehabilitation (117) 1. Introduction and Overview We are in need of 1 set of Parallel Bars for the New Riverhead PM&R PT Clinic. This piece of equipment is a necessary requirement in the treatment of veterans suffering from balance, coordination, gait problems as well as diagnoses related to a variety of neurologic, orthopedic or medically debilitating conditions. This will serve a majority of our patients in the New Clinic in Riverhead. 1.1 Background Due to the creation of the New Riverhead CBOC PT clinic we need to acquire the needed equipment to treat the veterans who will be attending therapy in these clinics. 1.2 Scope of Work Objectives The primary objective is to procure 1 set of electronically controlled parallel bars with the following features: 600 lb. weight capacity Height & weight adjustable Fingertip controls Tapered platform extensions Steel frame Bar Range: Horizontal: 15.5 24.5 Vertical: 25.25 39.75 Width: Platform 48 Between Standards: 31.5 Electrical: 7.6A, 115V, 782W, hospital grade plug. 2. Tasks Deliver 1 Set Parallel bars Northport VA Medical Center, Warehouse, Building 36, 79 Middleville Road, Northport, NY. Assembly and validation of performance. Training of staff. 3.
