Requesting for the task chairs to be boxed and palletized by the company. Does not include installation. Objectives The purpose of this procurement is to obtain task chairs in vinyl for the Waco VA nurses stations and check-in areas. The vinyl task chairs: five-star base, pneumatic, knee tilt forward ang, adjustable arms, vinyl, high back, size B. Back angle, adjustable. Base finish/frame and outer shell finish, black umber. Casters for hard floor, vinyl composition tile, and luxury vinyl flooring. Outer shell finish, non- upholstered. Arm pad finish, black. Fabric Color guard vinyl in black. Adjustable arms adjust 4 inches vertically with arm pads that adjust ¾ inches horizontally and pivot outward and inward over a 25-degree range. Knee tilt swivel with forward angle that allows the user to select a horizontal or 5-degree forward seat angle; the seat angle can be locked in either position. Task chairs adjustable back has a 10-degree tilt range from an upright position. The seat depth is 16 ¾ inches; the seat height ranges from 16 inches to 20 ¾ inches, the distance from the outside of one arm to the other is 16 ½ inches wide, the height of the back is 44 ¾ inches maximum, and the seat width is 20 ½ inches. Task chairs rated up to 300 pounds. Task chairs to be boxed and palletized by the company. Waco Veterans Affairs (VA) medical Center 4800 Memorial Drive, Waco, TX 76710. Scope of Work DELEIVERY: All furniture to the Waco, Texas, Red Building, (enter gate 4 from Beverly drive) located at Waco VA, Doris Miller VA Medical Center 4800, Memorial Drive Waco, Texas 76711. Delivery shall occur Monday-Friday between the hours of 8:00 AM-8:30 am. Note there is a dock area with a lift. Please call Joe Clark at cell phone # 254-379-8139 to schedule a day for installation and to confirm the 8:00 am/8:30 am delivery time. Security The contractor will not have access to Patient Health Information (PHI) nor will they have the capability of accessing patient information during the services provided to the VA.
