A/C (Assist Control) b. SPONT (Spontaneous) c. SIMV (Synchronized Intermittent Mandatory Ventilation) the mandatory breath type of and SIMV breath can be PC, VC, or VC+ d. BILEVEL a mixed mode which overlays the patient s spontaneous breaths onto the breath structure for PC mandatory breaths. Two levels of pressure, PL and PH are employed. e. CPAP (Continuous Positive Airway Pressure) f. VC (Volume Control/Volume Assist Control) g. PC (Pressure Control/Pressure Assist Control) h.VC+ (Volume Control Plus) a mandatory, pressure controlled breath type that does not restrict flow during the inspiratory phase, and automatically adjusts the inspiratory pressure target from breath to breath to achieve the desired tidal volume despite changing lung conditions. Leak Sync is available in this mode. i.PS (Pressure Support) range: 0 cmH2O to 70 cmH2O j. VS (Volume Support) k. +NIV (Non-Invasive) available in SIMV and Assist Control l.TC (Tube Compensation) additional positive pressure delivered to the patient during spontaneous breaths to overcome resistance of the artificial airway. 2.0 Ventilator must be compliant with IEC/ISO 80601-2-12. Configuration allows for cleaning/prevention of gas entry into the ventilator. This feature is useful to reduce/eliminate Ventilator Acquired Pneumonia and prevent cross-contamination issues, i.e. must allow for cleaning of gas pathway in the ventilator in order to prevent infectious organisms (e.g. SARS or Ebola) from contaminating the integrity of the ventilator. 3.0 Included Occlusion Algorithm the ventilator detects severe patient circuit occlusions in order to protect the patient from excessive airway pressures, or from receiving little or no gas. Mechanical ventilation will still be delivered if this condition exists. This addresses inadvertent ventilator shutdowns due to high airway pressures from occlusions. 4.0 Ventilator configuration must come with a N100 rated expiratory filter which prevents bacteria and viruses from passing through the ventilator to the health care environment and cross contaminating clinicians and other patients. This meets CDC requirements for n100 rating: a. Internal and external inspiratory filter bacterial/viral filtration efficiency > 99.999% b. Internal and external inspiratory filter particle filtration efficiency > 99.97% retention of particles 0.3um nominal at 100L/min flow. This meets CDC requirements for n100rating. 5.0 Ventilator must have a mode that patient-triggered and inspiratory effort based which delivers positive airway pressure that is directly proportional to a patient s inspiratory effort during spontaneous breathing; i.e. if a ventilated patient desires a deeper breath, ventilator will respond by increasing support. Mode is patient-triggered and pressure based and allows the patient to receive whatever ventilation and breathing pattern support their neural control center deems to be best in order to facilitate patient synchrony and weaning. 6.0 Ventilator must have a large screen (Graphical User Interface, minimum 15 inches diagonal),with capability of large fonts to facilitate ease of visibility through closed doors of negative pressure/isolation rooms. Screen needs to rotate at least 170 degrees and tilt up to 45 degrees from vertical to facilitate use in all ICU rooms, including negative pressure isolation. 7.0 Ventilator display must offer multiple wave forms and loops on the same screen and be highly customizable and easily configurable. E.g. ventilator display must offer the following: a. Curves b. Loops c. Numeric parameters d. Alarm history e. Graphic trends 8.0 Ventilator display must offer multiple presets for graphics and patient data to facilitate ease of monitoring. 9.0 Ventilator must have re-chargeable Li-Ion hot-swappable batteries which support basic ventilator and compressor function. E.g. a. Ventilator must be able to fully operate, with compressor, on battery power for a minimum of 1 hour if the main AC power source fails or drops below approx. 90 volts without changing batteries. b. Hot-swappable batteries permit uninterrupted use of the ventilator in the event of a prolonged A/C power outage by allowing fresh batteries to be exchanged for depleted batteries while the ventilator is in use, and without compromising patient care. 10.0 Ventilator must have on-board compressor system to permit continuation of use in case of outage of hospital provided compressed air. 11.0 Ventilator must have configurable, smart alarms with visual recommendation on remediation 12.0 Ventilator alarms must allow for automatic elevation in high priority alarms decibel level increases to raise awareness of alarm severity in presence of a disconnect or other high priority alarms. a. Required alarm levels: i..Low Priority Alarm: 50 76 decibels ii. Medium Priority Alarm: 52 78 decibels iii. High Priority Alarm: 58 86 decibels 13.0 Ventilator monitoring and alarms must cover the following: a. Exhaled tidal volume Low/high b. Exhaled minute volume low/high c. Inspired tidal volume High d. Respiratory rate High e. Circuit pressure Low/high f. Apnea interval 10s to 60s or Off in CPAP 14.0 Ventilator must offer a secondary system that will support the patient in conditions where certain mechanical failures have occurred and a secondary status display that provides additional information related to power source, peek pressure, and Positive End Expiratory (PEEP). 15.0 Ventilator must incorporate a Stand-by mode which pauses ventilation when the patient is disconnected and preserves settings; auto-detects patient upon reconnection; and automatically resumes ventilation without the need to push additional buttons. 16.0 Ventilator must provide for either pressure-triggered or flow-triggered breaths. Pressure sensitivity must be 0.1 cmH2O to 20 cmH2O and Flow sensitivity must be 0.2 L/min to 20 L/min 17.0 Ventilator must have the following minimum performance characteristics a. Adjustable rise time: 1% to 100% b.Expiratory sensitivity: 1% to 80%; 1 L/min to 10 L/min with PAV+ c. Tidal volume: 5 mL to 2500 mL d. Peak inspiratory flow: 3 L/min to 150 L/min e. Plateau time: 0.0mto 2.0 seconds f. Inspiratory pressure: 5 to 90 cmH2O g. Inspiratory time: 0.2 to 8.0 seconds h.I:E ratio: 1:299 to 4:1 i. Expiratory time> 0.20 seconds j. PEEP Positive end expiratory pressure: 0 cmH2O to 45 cmH20 k. Apnea ventilation mandatory type: PC, VC l. Apnea peak inspiratory flow: 3.0 L/min to 150 L/min m.Apnea tidal volume: 25mL to 2500mL 18.0 Additional requirements: a. Unlimited education for staff during the warranty period b. BIOmed education/seminar (factory) c. All inclusive service included for at least three years d. Must provide continuity of care with smooth transition between current ventilator and new ventilator to allow for consistent patient care, reduce training time and learning curve for respiratory therapists and doctors, and reduce potential confusion related to functionality and terminology of ventilator settings. (JAN 2008) 852-219-10 VA Notice of Total Service-Disabled Veteran-Owned Small Business Set-Aside (a) Gray market items are Original Equipment Manufacturer s (OEM) goods sold through unauthorized channels in direct competition with authorized distributors. This procurement is for new OEM medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts) for VA Medical Centers. No remanufactures or gray market items will be acceptable. (b) Vendor shall be an OEM, authorized dealer, authorized distributor or authorized reseller for the proposed medical supplies, medical equipment and/or services contracts for maintenance of medical equipment (i.e. replacement parts), verified by an authorization letter or other documents from the OEM, such that the OEM s warranty and service are provided and maintained by the OEM. All software licensing, warranty and service associated with the medical supplies, medical equipment and/or services contracts for maintenance of medical equipment shall be in accordance with the OEM terms and conditions. (c) The delivery of gray market items to the VA in the fulfillment of an order/award constitutes a breach of contract. Accordingly, the VA reserves the right enforce any of its contractual remedies. This includes termination of the contract or, solely at the VA s election, allowing the Vendor to replace, at no cost to the Government, any remanufactured or gray market item(s) delivered to a VA medical facility upon discovery of such items. (ix) Evaluation of this requirement will be based on PRICE ONLY.
