Description

REQUEST FOR INFORMATION / SOURCES SOUGHT INQUIRY The Department of Veterans Affairs Network Contracting Office (NCO) 19, Rocky Mountain Acquisition Center (RMAC) is seeking information and sources that can provide on-site Optometry services for its Veteran population at the Sheridan Healthcare System (hereinafter referred to as SVAHCS). Place of Performance: Services shall be provided on site, SVAHCS, 1898 Fort Rd., Sheridan WY 82801 Qualifications: The Contractor shall furnish all personnel to provide services necessary to perform onsite Optometry Services to eligible beneficiaries of the SVAHCS. The Contractor provider(s) care shall cover the range of Optometry services as would be provided in a state-of-the-art civilian medical treatment facility and the standard of care shall be of a quality, meeting or exceeding currently recognized national standards as established by the American Academy of Optometry http://www.aaopt.org/ License Contractor provider(s) assigned by the Contractor to perform the services covered by this contract shall have a current license to practice medicine in any State, Territory, or Commonwealth of the United States or the District of Columbia when services are performed onsite on VA property. All licenses held by the personnel working on this contract shall be full and unrestricted licenses. Contractor s provider (s) who have current, full and unrestricted licenses in one or more states, but who have, or ever had, a license restricted, suspended, revoked, voluntarily revoked, voluntarily surrendered pending action or denied upon application will not be considered for the purposes of this contract. Board Certification - All Contractor s provider(s) shall be by the American Board of Optometry http://www.americanboardofoptometry.org/, and be currently certified in Basic Life Support (BLS). All continuing education courses required for maintaining certification must be kept up to date at all times. Documentation verifying current certification shall be provided by the Contractor to the VA COR on an annual basis for each year of contract performance. Type of Contract: If this request for information leads to a future solicitation, the Government intends to award a firm-fixed price contract. The performance period shall be one (1) base year with four (4); one (1) option years. This is a request for information and sources only, which may or may not lead to a future solicitation! This is not a request for proposal (RFP). The VA will not pay for any information received resulting from this sources sought notice. Requests for copies of a solicitation shall not be honored or acknowledged. Information should be forwarded to the Contract Specialist. Because this is a request for information only, answers to questions will not be posted.
