(See attachment on Fed Biz Ops) As prescribed in 215.408(5), use the following provision: PROPOSAL ADEQUACY CHECKLIST (JAN 2014) The offeror shall complete the following checklist, providing location of requested information, or an explanation of why the requested information is not provided.
