- This is a sources sought notice 36C24418Q9248; this is neither a solicitation nor a promise of future work. Philadelphia VAMC, (642) is seeking SDVOSB/VOSB and any SBC regardless of Socio-economic category that can provide Bariatric exam tables. The exam tables being sought must have a weight capacity of 650 lbs. The tables must include built in scales. The tables should be adjustable beyond the ADA requirement of 17 inches. They should adjust to 15.5 uncompressed and 14.6 compressed. The above requirements are cited because they best meet the needs of the various VA patients and their medical apparatus to provide the best care possible. If the exam tables you are offering meet these details so note and illustrate in your response. The items should be delivered to the Philadelphia VAMC. The veteran business must be registered in SAM and certified in VETBIZ (VIP). The small businesses must be registered in SAM. All Vendors interested in, and with the capability and capacity to fulfill this requirement, are required to furnish the following minimum information in reply to this sources sought: Company Name,, address, Government Business Point of Contact, telephone number and email address. Company DUNS number, CAGE Code, and Tax ID numbers. Identify any current Federal Supply Schedule, GSA Contracts held, if none then state that no FSS/GSA schedules are held. Responses shall be submitted electronically, via email to Donna Toliver at donna.toliver3@va.gov due no later Thursday 06/14/2018 at 3:00 pm.
