Description
This is a Sources Sought Notice (a) The Government does not intend to award a contract on the basis of this Sources Sought or to otherwise pay for the information solicited. (b) Although "proposal," "offeror," contractor, and "offeror" may be used in this sources sought notice, any response will be treated as information only. It shall not be used as a proposal. (c) Any information received from a contractor in response to this Sources Sought may be used in creating a solicitation. Any information received which is marked with a statement, such as "proprietary" or "confidential," intended to restrict distribution will not be distributed outside of the Government, except as required by law. (d) This Sources Sought is issued by VISN 2 Contracting Office in Albany, NY for the purpose of collecting information about procurement of Negative Pressure Total Exhaust Aseptic Compounding Containment Isolator at Brooklyn VAMC 800 Poly Place Brooklyn, NY 11209. The VA is seeking to provide for this requirement as of 20 days after receipt of award. The NAICS code identified for this requirement is 337127 Laboratory Type Furniture Manufacturing. The requirement is detailed in the Statement of Work section of this document. (e) Cursory market research has identified several small businesses that may be able to provide for this requirement. Contractors that are able to meet the requirements in the attachment are encouraged to email a capability statement and full information to Dayna Dickson at Dayna.Dickson@va.gov - Part of the purpose of this sources sought is to determine the viability of set aside to a specific socio-economic category. Contractors shall identify the NAICS code for the services being offered as well as their size status under said NAICS. If contractor is a Service Disabled Veteran Owned Small Business (SDVOSB), a Veteran Owned Small Business(VOSB), or any other special category of contractors, then contractor is responsible for providing documentation in support of this. This information will be used to determine the viability of a set aside for this requirement. - Contractors shall also identify any Federal Supply Schedules that may carry the desired services. - Contractors shall identify pertinent point of contact for company, contractor DUNS number for size standard and socioeconomic verification in SAM and VIP. - Contractors shall include any relevant comments about the Attachment(s) if applicable. Pharmacy Negative Pressure Isolation Hoods Scope of Work Request is for purchase of two Nuaire negative pressure total exhaust aseptic compounding containment isolator hood. Negative pressure Isolators are a crucial part of Pharmacy operations and enable pharmacists to compound hazardous IV products such as chemotherapy for critically ill patients and those that are unable to take medications by mouth. The purchase of two Nuaire negative pressure total exhaust aseptic compounding containment isolator hood will allow for Pharmacy to continue preparation of IV chemotherapeutic products and BCG injections. The primary compounding isolator safety alarm is not working and no parts are available from the manufacturer. As stated earlier, due to other construction projects and limited parking available at the site, contractor must coordinate any parking needs with the assigned COR and the Police Service. Details of any vehicles entering the site must be provided in advance. (Driver s Full Name, Driver s License Number, Vehicle Description, Vehicle License Plate Number) Government-Furnished Property Contractor will not need any government furnished equipment or property. Education and Training Requirements The Vendor shall be responsible for ensuring that vendor employees providing work on the required equipment are fully trained and completely competent to perform the required work. Following the guidelines so of USP 797 and USP 800. VA Service Point of Contacts List the facility point of contacts responsible for receipt and installation of equipment. Include property management and biomed for property inventory and incoming inspection. Limited Source Justification/Recommended Source Nuaire is recommended vs Baker due to time constraints. The current negative pressure hazardous hood safety alarm is not functional and the manufacturer doesn t support this model due to age. Nuaire has a lead time of 30-60 days vs Baker has a lead time of 16-20 weeks. VA Patient Privacy and Network Infrastructure None of the equipment listed within this procurement package will access the VA Network or store any sensitive patient information. Any contractors that will come on site for install will not need access to VA network. All contractors must comply with VA regulation and complete the following training: VA Privacy Training for Personnel without Access to VA Computer Systems or Direct Access or use to VA Sensitive Information. Reference the attached Handbook 6500.6 Appendix A - Checklist for Information Security in the initiation phase of acquisitions. The system does not transmit or maintain electronic Protected Health Information. A Manufacturer Disclosure Statement for Medical Device Security (MDS2) form in not applicable. A VA Directive 6550 Pre-Procurement Assessment form is not applicable. Recalls The Vendor shall immediately notify all affected VA Medical Centers and designated POC in the event of a device recall, removal, required adjustment, or required modification that is suggested or mandated by the Vendor, distributor, manufacturer, or any regulatory or government agency. The following information shall be included in this notification: A complete item description and identification (to include lot and serial numbers where applicable); Administrative identification data including order numbers and order date; Reasons for recall, removal, adjustment, or modification; and Instructions for appropriate corrective action.
