Description

1. PURPOSE: This amendment is issued in order to provide a revision to the previous format for obtaining additional information and to extend the response period for this announcement accordingly. 2. REQUESTED INFORMATION: The Planner is seeking additional information necessary to identify potential offerors to fulfill the capabilities represented in this notice. Respondents must furnish the requested information to the Requirements Planner for this notice utilizing ONLY the provided (.xlsx) Additional Information Request. Complete columns A thru I for each item accordingly. 3. CONTACT INFORMATION: Responses to this amendment must be submitted to both the Planner and alternate point of contact, via e-mail. The subject line of email responses to this amendment shall be annotated as such: Your Company Name SS Amendment 4 Additional Information. To: The Requirements Planner: Charles.Herring@va.gov Cc: Alternate POC: nicole.eger@mcix.com 4. LATE SUBMISSIONS: Failure to respond to this amendment will not preclude participation in any future competition, nor will information provided in response to this amendment be used in any way to exclude anyone from responding to any future requests for proposals/quotes. Communications with the Planner with regard to this amendment will only be permitted in writing during the amendment response period. Responses to the amendment received by the Planner after the submittal deadline and time indicated may be considered for acceptance on a case by case basis if such consideration is requested by the respondent prior to the response due date. 5. OWNERSHIP OF RESPONSES TO THIS NOTICE: All informational material submitted in response to this request becomes property of the Planner. 6. RELEASE OF CLAIMS, LIABILITY, AND PREPARATION EXPENSES: Under no circumstances shall the Planner or ANY of its affiliates be responsible for any response preparation expenses, submission costs, or any other expenses, costs, or damages, of whatever nature incurred as a result of the Respondent's participation in this process. Respondent understands and agrees that it submits its response at its own risk and expense, and releases the Planner or ANY of its affiliates from any claim for damages or other liability arising out of this notice. 7. ERRORS IN RESPONSE: The Planner shall not be responsible for correcting any errors in the Respondent's responses. The Respondent is responsible for the careful review of its entire response to ensure that all information is correct and complete. Respondents are responsible for correcting all errors or omissions contained in their responses prior to submission. 8. ADDENDUM: The Planner reserves the right to issue an addendum to this notice, by means of amendment, at any time for any reason; to include extending the associated due dates.
