Description
This is a Sources Sought Notice (a) The Government does not intend to award a contract on the basis of this Sources Sought or to otherwise pay for the information solicited. (b) Although "proposal," "offeror," contractor, and "offeror" may be used in this sources sought notice, any response will be treated as information only. It shall not be used as a proposal. (c) Any information received from a contractor in response to this Sources Sought may be used in creating a solicitation. Any information received which is marked with a statement, such as "proprietary" or "confidential," intended to restrict distribution will not be distributed outside of the Government, except as required by law. (d) This Sources Sought is issued by VISN 2 Contracting Office in Albany, NY for the purpose of collecting information about procurement of custome mobile bed rack Northport VA Medical Center, Northport NY. The VA is seeking to provide for this requirement as of 20 days after receipt of award. The NAICS code identified for this requirement is 339113 furniture, hospital, specialized. The requirement is detailed in the Statement of Work section of this document. (e) Cursory market research has identified several small businesses that may be able to provide for this requirement. Contractors that are able to meet the requirements in the attachment are encouraged to email a capability statement and full information to Dayna Dickson at Dayna.Dickson@va.gov - Part of the purpose of this sources sought is to determine the viability of set aside to a specific socio-economic category. Contractors shall identify the NAICS code for the services being offered as well as their size status under said NAICS. If contractor is a Service Disabled Veteran Owned Small Business (SDVOSB), a Veteran Owned Small Business (VOSB), or any other special category of contractors, then contractor is responsible for providing documentation in support of this. This information will be used to determine the viability of a set aside for this requirement. - Contractors shall also identify any Federal Supply Schedules that may carry the desired services. - Contractors shall identify pertinent point of contact for company, contractor DUNS number for size standard and socioeconomic verification in SAM and VIP. - Contractors shall include any relevant comments about the Attachment(s) if applicable. STATEMENT OF WORK (SOW) Mobile Cot Storage 632-18-3-358-0184 Northport VA Medical Center, Northport NY (632) Emergency Management (001) Associate Director s Office Introduction and Overview These mobile storage units will ensure the facility has an efficient process for deploying shelter in place cots for mission essential staff and enhance the recovery reconstitution process. This is an improvement item that currently is a gap in the facilities SIP capability to provide a safe and secure environment for mandated mission essential staff. 1.1 Background The facility is required to disinfect, maintain and store the Shelter In Place cots after each use. It has been determined that mobile storage units will enhance the longevity of the cots and allow for easy deployment from the storage room to the clinical floors when activated. Currently there are no mobile storage units for the facility cots and the integrity of the cots has been compromised. The mobile storage units will keep the cots sanitary and sound during deployment, reconstitution and storage situations 1.2 Scope of Work A customizable cot rack for storing up to 30 cots depending on requested dimensions and size of cots to be stored. 1.3 Objectives The primary objective for the procurement of 10 mobile storage racks is to protect the shelter in place cots and provide easy deployment to the identified sleeping locations for mission essential staff during activation of the emergency operations plan. The second objective would be to ensure the cots are kept in a clean secure location to ensure their sustainability over the period of multiple years. Requirements / Specifications Current quote is based on custom built measurements for a 30 cot rack (extra tall) - 86" (with casters) x 48" x 36.5", 2 shelves in addition to the bottom and open top, to accommodate storage and deployment of the Facilities shelter in place cot inventory. Materials: 1 x 14 gauge square tubes 14 gauge plate steel; Welded steel construction 1/8" x 1/2" flat bar; Laser cut components Caster plates for mounting casters Black powder coating of Cart (2) 3" swivel casters with gray wheels; (2) 3" swivel casters with locking (3) 1 x 10 Strap with Cambuckle and S-hooks Baked powder coated textured finish Dust Cover Removable nylon to ensure stability of cots Production: Depending on the Cot other customer restrictions: the square tube 1 x 14 gauge steel is used for the 4 corner posts and for the top and bottom edges as well as the shelf supports for stability 14 gauge plate steel is used as the bottom and any additional shelves 1/8 x ½ flat bar is used as supporting brace bars mounted either vertically or horizontally between the corner post for keeping the cots on the shelves (dependent on design) Caster plates are mounted to the bottom of the cart Cart is Powder coated Casters are mounted to the caster plates Holes are drilled for the attachment of the S-Hook Straps for the best configuration to support the cots 1-Year Limited Statement of warranty of equipment, minimum one year. Statement of service included or for purchase: preventive maintenance; service and repair.
