- This is a Sources Sought Notice (a) The Government does not intend to award a contract on the basis of this Sources Sought or to otherwise pay for the information solicited. (b) Although "proposal," "offeror," contractor, and "offeror" may be used in this sources sought notice, any response will be treated as information only. It shall not be used as a proposal. (c) Any information received from a contractor in response to this Sources Sought may be used in creating a solicitation. Any information received which is marked with a statement, such as "proprietary" or "confidential," intended to restrict distribution will not be distributed outside of the Government, except as required by law. (d) This Sources Sought is issued by VISN 2 Contracting Office in Albany, NY for the purpose of collecting information about procurement of stainless steel triple bay sink with reverse osmosis/distilled faucet for Northport VA Medical Center, Northport NY. The VA is seeking to provide for this requirement as of 20 days after receipt of award. The NAICS code identified for this requirement is 332999 all other miscellaneous fabricated metal product manufacturing. The requirement is detailed in the Statement of Work section of this document. (e) Cursory market research has identified several small businesses that may be able to provide for this requirement. Contractors that are able to meet the requirements in the attachment are encouraged to email a capability statement and full information to Dayna Dickson at Dayna.Dickson@va.gov - Part of the purpose of this sources sought is to determine the viability of set aside to a specific socio-economic category. Contractors shall identify the NAICS code for the services being offered as well as their size status under said NAICS. If contractor is a Service Disabled Veteran Owned Small Business (SDVOSB), a Veteran Owned Small Business (VOSB), or any other special category of contractors, then contractor is responsible for providing documentation in support of this. This information will be used to determine the viability of a set aside for this requirement. - Contractors shall also identify any Federal Supply Schedules that may carry the desired services. - Contractors shall identify pertinent point of contact for company, contractor DUNS number for size standard and socioeconomic verification in SAM and VIP. - Contractors shall include any relevant comments about the Attachment(s) if applicable. Statement of Work Washer Disinfector for SPS Department 632-18-3-3086-0207 Provide and install one Triple Bay Stainless Steel Work Sink Reverse Osmosis/Distilled Faucet for Northport VA Medical Center, Bldg. 200 1st Floor SPS Area. This project involves the purchase of one (1) Triple Bay Work Sink. This project is designed to ensure standards and practices are followed. In adhering to the mission Statement of the Northport VA, it is our duty to provide exceptional care for our Veterans. Our surgical patients rely on the quality of our Medical Center from pre-admission testing to the operating room and recovery period. Hospitals and health chiefs have the responsibility to ensure that every piece of equipment is worthy of the faith placed in it. This request exemplifies our commitment to offering efficient, safe integrated quality healthcare capable of providing a full range of state-of-the-art surgical and perioperative care in an organization that is readily accessible and responsive to change. The C&A requirements do not apply and a Security Accreditation Package is not required. Background This system is requested for use in the Northport VA Sterile Processing Decontamination area. Scope of Work. The Triple Bay Work Sink must contain and apply as follows: Stainless steel construction. (3) 28 x 16 x 10.5 deep sink bowls. (3) Lever handle drain control valves. (3) Deck mounted hot/cold water faucets with wrist blade controls. Deck mounted hot/cold water spray rinse. Back splash. Raised marine edge for retaining water on work surface. Shelving area on left or right side (opposite sink bowls) DI/RO water faucet. Selecta water pistol. Selecta air pistol. Set of (8) pistol attachments for Selecta pistols. Etched gallon marking in sink bowl. Three Flushing pumps with tubing Sink Auto Dispenser with Unique Wave Switch Touch-less Technology. Objectives: The new triple bay work sink will provide a larger bay needed for soaking to be compliant with manufacturer guidelines. Larger bays will allow SPS to soak total joint trays as needed and will cut down on manual scrubbing. 2. Requirements Equipment to be delivered to medical center. Training of staff, minimum one person, to be performed on site. Statement of warranty of equipment, minimum one year. Statement of service included or for purchase: preventive maintenance; service and repair. 2. 1 Tasks Deliver one hydraulic 3 bay sink to Building 36, warehouse, 79 Middleville Road, Northport, NY. Validation of performance by vendor within 30 days after installation. Training of staff. Schedules/Milestones Delivery of one 3 bay sink within 30 days of purchase. Validation of performance within 30 days of delivery. Transmittal/Delivery/Accessibility The contractor shall provide [1] hard copy of each deliverable and one electronic version as well as user s manual.
