- STATEMENT OF WORK WEST PHILADELPHIA CLINIC UNION INSTALLATION 5/29/18 General Project Information: The Corporal Michael J. Crescenz Department of Veterans Affairs Medical Center (CMCVAMC) is currently renovating space for the West Philadelphia Clinic at 63rd and Market Street in Philadelphia, PA. The CMCVAMC is looking for a company to provide Comprehensive equipment delivery Service by Union installers. It is the intent of this project that the Purchase Order will be issued to one vendor. It is the intent of this project to provide the following: Pick up all equipment and approximately 75 boxes of office supplies from the CMCVAMC at 3900 Woodland Ave, Phila, PA 19104 and deliver to 6232 Market St., Phila, PA. There is a list of the equipment on a separate sheet. The scope of work includes pick up at the CMCVAMC and delivery of the items listed by Union installers. The items you are picking up should be placed in the rooms that are marked on each item. The CMCVAMC Interior Designer and Nurse Manager will be on site to show the installers exactly where everything will need to be placed. The West Philadelphia Clinic building does not have a loading dock so delivery trucks with a lift gate will be required. Union Installers must laydown protective covering on the floor at the delivery entrance and thru the site while delivering the equipment, and remove it when the install is complete. All boxes etc containing equipment must be opened and place in the designated rooms. All trash is to be removed off the premises and disposed of off-site.
