- Description
- The Department of Veterans Affairs, Veterans Integrated Service Network (VISN 6) has a need for Home Respiratory Supplies to include storage, delivery, set-up, adjustment, patient/caregiver instructions, equipment servicing and recovery of equipment within the VISN 6 area which includes the following seven medical facilities and outlying areas: Asheville, North Carolina; Durham, North Carolina; Fayetteville, North Carolina; Hampton, Virginia; Richmond, Virginia; Salisbury, North Carolina; and Salem, Virginia. VISN 6 currently serves an estimated 5,000 oxygen using beneficiaries covering two (2) states: North Carolina and Virginia. Home Oxygen delivery and set-up shall be available 24 hours a day, seven (7) days a week, including holidays. The Contractor must be Joint Commission (JC) accredited for Home Care or equivalent accredited standard and must maintain the certification or equivalent accredited standard for the duration of the contract. Contractors shall have applicable expertise in government acquisition and program management disciplines. Services shall include providing transportation of equipment, services, labor, supervision, patient education, safety management, infection control, as necessary for beneficiaries receiving Home Oxygen services as specified under this Request for Proposal (RFP). Adequate personnel staffing and warehouse capabilities are required to properly maintain equipment and render services throughout the service areas noted above in accordance with all terms and conditions of the RFP for the duration of the performance period. Companies will be responsible for coordinating, supervising, monitoring, and evaluating the care and service provided and shall assure that contracted services are performed per the terms of the contract. Companies will also be responsible to assure their personnel meet the requirements of the contract and are competent to do the jobs assigned to them. Government monitoring will focus on timeliness of equipment delivery; timeliness of repairs or maintenance; accuracy of required documentation submitted, and adherence to Joint Commission standards. This requirement is a 100% Set-Aside for Service Disabled Veteran Owned Small Businesses (SDVOSB) under NAICS Code 532283. Size Standard is 32.5M. Business submitting proposals must be registered in System for Award Management (SAM) and must be registered in VetBiz at http://www.VetBiz.gov. A single award will be made and will include a Base Year with four (4) one (1) Option Years periods to begins on or around January 01, 2019.
