- Laboratory Specifications Pricing Schedule - Excel Pricing Schedule - PDF **TOTAL SMALL BUSINESS SET ASIDE** This notice is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a one - time procurement order for several Laboratory equipment at Chinle Comprehensive Health Care Facility, Chinle, Arizona. CCHCF identified a specific brand name, but will accept brand name or equal descriptions per FAR 6.302-1(c)(2). CCHCF intends to award a firm fixed price contract(s) as a result of this solicitation.f CCHCF is based in Chinle, Arizona (Northeast Arizona near Canyon De Chelly National Monument). The CCHCF is a 60 bed hospital which serves as the health care hub for the region. The medical staff includes Family Physicians, Internists, Pediatricians, General Surgeons, OB/GYN's, Anesthesiologists, and a Psychiatrist. In addition to routine outpatient and inpatient primary care, services available to our patients include: Adult Intensive Care, General Surgery (including laparoscopic surgery), routine and operative Obstetrics, and 24-Hour Emergency Room Services. Health care services are provided to approximately 37,000 active users. EVALUATION CRITERIA: The Government reserves the right to issue a single award or multiple awards to the contractor(s) whose quote represents the best value as defined by FAR 2.101. In determining best value, the following Evaluation Factors will be considered: Product Specifications/Technical Capabilities, Past Performance, Warranty, Logistics, and Delivery. The Evaluation Factors and significant sub factors when combined are significantly more important than cost or price. The socio - Economic status of an offeror may also be considered, should one or more quotes represent the best value. SUBMITTAL PROCESS: Email offer directly to the Contracting Officer for this action is: Ashleigh Yazzie, Purchasing Agent, 928-674-7801; ashleigh.yazzie@ihs.gov. Limit offer size to 6MB. Due to urgent need there will be no discussions regarding this solicitation. To be considered for award, your offer shall include the following documents: 1) Pricing: Complete and sign the attached Pricing Schedule. The Government will consider competitive rates upon negotiation. 2) Provide your own Quotation 3) Past Performance: Provide three (3) references, name, title, organization name, telephone number, and email address. 4) Experience: Provide evidence in experience in providing similar services to Federal Government or other large commercial/government entities within the last three (3) years of operation. 5) Specifications/Technical Capabilities: If providing an equal product, then provide your product specifications.
