Description
Request for Information Template Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. Quotes will be evaluated and awarded to the offeror with the lowest price technically acceptable. The Government reserves the right to award on a multiple award or an all or none basis. All firms must be registered in the System for Award Management database @ www.sam.gov and shall be considered for award. This combined synopsis/solicitation is issued as a Request for Quote (RFQ) W91SMC-18-Q-7003. Submit written offers in accordance with (IAW) the Contract Line Item Number (CLIN) structure outlined in announcement. Oral offers will not be accepted. All firms or individuals responding must be registered with the System for Award Management (SAM) (https://www.sam.gov). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-98 (effective 31 May 2018). This acquisition is being conducted as a 100% Small Business Set-Aside IAW FAR 19.502-2(b). North American Industrial Classification Standard (NAICS) 722310 "Food Service Contractors", and Size Standard of $38.5M applies to this procurement. RFQ Submission For Food Service Attendants: •1. Quotes should conform to the CLIN structure as established in RFQ Submission Format. •2. Quotes shall be valid for a minimum of 60 days. •3. Please see attached Performance Work Statement (PWS) for detailed description of requirements. •4. Quotes must be submitted to CMSgt Brent Keller and SrA Rachel Clark via email at brent.d.keller.mil@mail.mil and rachel.e.clark18.mil@mail.mil NLT 12:00 PM CST, 11 July 2018. •5. The resulting contract will be Firm-Fixed Price (FFP) and the Period of Performance is for one base year (August 4, 2018- August 3, 2019) with four follow-on option years. •6. All questions should be submitted by email to the contracting office no later than close of business on 02 July, 2018, using the Request for Information (RFI) template provided. CLIN 0001 Food Service Attendants QTY 1 Job The contractor shall, except as specified in this Performance Work Statement (PWS) as government furnished property or services, provide all personnel and supervision necessary to perform mess attendant services at 183 d Wing Air National Guard Base to help with lunch meal service one weekend per month. Please reference the attached PWS for more details. Period of Performance from August 4, 2018- August 3, 2019. CLIN 0002 Contractor Manpower Reporting QTY 1 Job In Accordance with Sections 235 and 233a of Title 10, United States Code, the contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Illinois Air National Guard via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address: http://ecmra.mil. CLIN 0003 Food Service Attendants - Option Year 1 QTY 1 Job The contractor shall, except as specified in this Performance Work Statement (PWS) as government furnished property or services, provide all personnel and supervision necessary to perform mess attendant services at 183 d Wing Air National Guard Base to help with lunch meal service one weekend per month.
