Page 1 of 5 THIS IS A SOURCES SOUGHT ANNOUNCEMENT ONLY. This is not a solicitation for proposals, proposal abstracts, bids, or quotations. The purpose of this notice is to obtain information regarding the availability and capability of all qualified sources to perform a potential requirement. Responses to the information requested will assist the Government in determining the appropriate acquisition method. All Contractors submitting a response to this Sources Sought will have an established business with three (3) years of consecutive experience in performing the Inspections, Testing, Reporting, and Annual Maintenance of installed Simplex Grinnell Fire Alarms and Fire Suppression Systems, in accordance with National Fire Protection Association (NFPA) 72. This will require 24/7 monitoring by a contractor qualified to perform these services, while maintaining the warranty.. The Contractor shall furnish all labor, supervision, materials, tools, and equipment necessary to complete the required services listed below in draft Performance Work Statement (PWS). The following NAICS Code and Small Business Size Standard has been identified for this requirement: 561621 Security System Services (except Locksmiths) Standard of $20.5 Million in annual revenue. Interested parties shall provide a brief overview of their firm s capabilities and past experience with projects similar in size and scope of this project. In applicable certifications and/or accreditations to include, at a minimum, the EPA Universal Certification as required under EPA Regulation 40 CFR Part 82, Subpart F, Section 608. Capability statements shall also include: DUNS number Organization Name and Assumed Name/dba (if applicable) Organization Address Point of Contact (including name, title, addresses, telephone number, fax number and Email address) Business size (small business or other than small business) Socio-economic status (If SDVOSB or VOSB provide SDVOSB or VOSB CVE verification) (All information shall be submitted in Word or PDF format, not to exceed 10 pages, including all attachments.) Firms shall identify whether they are a small business or other than small business in accordance with the size standard for NAICS code 561621. If another NAICS code may be applicable, please identify the NAICS in your capability statement along with your corresponding socio-economic status. All information must be received no later than Monday, June 18, 2018 at 10:00 AM (CST). Email all information to Christopher Cook, Contract Specialist at Christopher.cook6@va.gov. The Government will evaluate market information to ascertain potential market capacity to provide services consistent in scope and scale with those described in this notice and otherwise anticipated. Telephone inquiries will not be accepted or returned. Based on the responses to this sources sought notice/market research, this requirement may be set-aside for small businesses, sole sourced, or procured through full and open competition. Therefore, we recommend that you monitor the FBO website for all information relevant to desired acquisitions. PERFORMANCE WORK STATEMENT/SPECIFICATIONS 1.0 DESCRIPTION OF SERVICES: This requirement is for the Inspections, Testing, Reporting, and Annual Maintenance of installed Simplex Grinnell Fire Alarms and Fire Suppression Systems at CAPT Lovell Federal Health Care Center (FHCC), in accordance with National Fire Protection Association (NFPA) 72. This will require 24/7 monitoring by a contractor qualified to perform these services, while maintaining the warranty. 2.0 GENERAL REQUIREMENTS: 2.1 All work under the provisions of this contract must be performed by level II NICET factory certified fire alarm sys tem Service Technicians directly employed by the contractor, who are trained and experienced in the servicing of the exact types and manufacture of the SimplexGrinnell systems and devices installed. Must have access and knowledge of Simplex Grinnell proprietary systems programing. 2.2 The services under this agreement shall include the work defined in Attachments A & B. All services shall be performed on site at the Lovell FHCC in North Chicago, IL. 2.3 While performing work under the provisions of this contract, the contractor s employees must strictly observe the requirements of all applicable Occupational Safety & Health Administration, Environ mental Protection Agency, Department of Veterans Affairs rules, regulations & policies, particularly those pertaining to NFPA, JACHO, infection control, lock out/tag out procedures and confined space entry. 2.4 The absence or omission of an existing alarm system, component, or device from the Device List will not relieve the contractor of responsibility for inspecting, testing, repairing, servicing and maintaining that system component or device in accordance with the requirements of this Statement of Work. 2.5 Existing specified alarm and system components or devices found to be in failure mode or otherwise identified by FHCC personnel as requiring repair or replacement, will be reported to the contractor for appropriate corrective action. 2.6 The contractor s Service Technician must coordinate with the Chief FMS, FHCC Police, or COR, then proceed directly to the job site to begin the necessary work. Troubleshooting, diagnosis, or re pair of existing specified fire alarm, security alarm, or monitoring system compo nents will be performed between 7:00AM to 4:30PM, Mon day through Friday excluding Federal holidays. How ever, in the event of a fire alarm system fail ure affecting more than one fire zone, or when otherwise considered necessary by Chief FMS, the contractor will be required to provide emergency repair service as nec essary to correct the problem at any time of the day or night, as well as weekends & Federal holidays. 3.0 TESTING 3.1 Within five (5) calendar days of receipt of notice of award, the contractor must provide the Chief, FMS and COR with the phone number(s) at which requests for emergency repair service as specified at paragraph 2.6 above will be accepted. The phone number specified shall not be to a voice mail box, an answering machine, or answering service. The COR will provide the contractor with a list of the names & titles of FHCC employees authorized to initiate requests for repair services. 3.2 Prior to beginning any work under the provisions of this contract on any given day, the contractor s Service Technician must report to Chief, FMS or COR, either in person or by phone and advise him or her as to the nature & location of the work to be performed that day. COR will nor mally assign someone to accompany the Service Technician to observe the work in progress. Upon completion of the work, the Service Technician must provide a detailed written re port to the COR, completely de scribing the specific main tenance service completed, including device identification, location, total hours worked, and a de scription of all items requiring further attention, if any. 3.3 The contractor must inspect and test all specified fire alarm control panels in accordance with ap plica ble laws and NFPA codes annually. Con tractor must ensure that the accomplishment of each inspection is closely coordinated with the COR or Chief, FMS. The contractor must provide the Chief, FMS within ten (10) work days of com pletion with a detailed written report of the results of the inspection & testing of each fire alarm con trol panel. 3.4 The contractor must perform cleaning and sensitivity testing of all specified smoke detectors in ac cordance with applicable laws and current NFPA Codes annually. The contractor must provide the Chief, FMS with a detailed written report of the results of sensitivity testing within ten (10) work days of com pletion of sensitivity testing. 3.5 The contractor must inspect and test Physical Security and environmental monitoring alarm control panels annually. Contractor must ensure the accomplishment of each inspection is closely coordi nated with the Police Service, FMS or COR. Contractor must provide the Chief, FMS within ten (10) work days of completion with a detailed written report of the results of the inspection & testing of each alarm control panel. 3.6 The contractor must inspect and test all specified fire alarm panels, smoke detectors, pull stations, heat detectors, and water flow & tamper switches. The contractor must ensure that the accomplish ment of each inspection is closely coordinated with Chief, FMS or COR. The contractor must pro vide the Chief, FMS or COR within ten (10) work days of completion with a detailed written report of the re sults of the inspection and testing of each fire alarm. 4.0 GENERAL INFORMATION: 4.1 CONTRACTING OFFICER REPRESTATIVE (COR): All services shall be performed at the CAPT James A. Lovell Federal Health Care Center (FHCC) and may be ac complished as scheduled in advance, and mutually agreed upon between the Contractor and COR, George Walsh: (224) 610-3121. Notwithstanding, the COR shall receive a written report of services performed upon completion of each occurrence via email to: george.walsh@va.gov. 4.2 HOURS OF OPERATION: Normal business hours are: 7:00 AM to 4:30 PM, Monday through Friday, excluding National Holidays:   New Years Day Martin Luther King Presidents Day Labor Day Memorial Day Independence Day Columbus Day Thanksgiving Christmas Veterans Day 4.3 ACRONYMS: AOD Administrator on Duty CO Contracting Officer COR Contracting Officer s Representative OSHA Occupational Safety & Health Administration. FHCC Federal Health Care Center EPA Environmental Protection Agency QASP Quality Assurance Surveillance Plan PWS Performance Work Statement JACHO- The Joint Commission National Institute for Certification in Engineering Technologies 4.4 Contractor personnel shall perform work in a professional manner; present a neat appearance; & wear visible identification always while on FHCC premises. Contractor personnel shall be easily rec ognized as contractor employees (e.g. wear distinctive clothing such as a uniform, badge, patch, etc.). 4.5 The Contractor shall be responsible to ensure Contractor employees providing work on this contract are fully trained and completely competent to perform the required work. 4.6 The Contractor shall be responsible to ensure that Contractor employees coming to the work site will receive the information required above 5.0 QUALITY ASSURANCE: Contractor shall ensure services are performed in accordance with accepted commercial practices. The Government will evaluate the Contractor s performance & approve all services per the Chief, FMS or designee in accordance with the Quality Assurance Surveillance Plan (QASP). 6.0 SECURITY REQUIREMENTS: All contractor personnel working onsite at FHCC shall check in and out at Building 188 during normal working hours and after normal working hours at the Quarterdeck located at the front entrance of the main hospital (Bldg. 133EF). It is the responsi bility of the Contractor to park in a designated parking area. Information on parking is availa ble from FHCC Police or the COR. Access to work areas & performance monitoring will be accom plished by FMS personnel. Smoking is prohibited inside all FHCC buildings. Possession of weapons is prohibi ted. Enclosed containers, including tool kits, shall be subject to search. Violations of FHCC regulations may result in citation an swerable in the U.S. (Federal) District Court, not a local, state, or municipal court. 7.0 COMPLIANCE: During work execution, Contractor shall take special care to protect Government prop erty & any property where services will be performed. Damage resulting from Contractor operations shall be repaired by the Contractor at no additional cost to the Gov ernment. Contractor shall be fully responsi ble for compliance with all Federal, VA, FHCC, state, and local environmental/occupational safety laws, rules & regulations. The contractor shall follow life and safety codes, and take necessary actions to avoid conditions that may be hazardous to the health & safety of hospital patients and staff. The contractor is responsible for meeting all applicable OSHA and FHCC guidelines for safe working processes. Contrac tor shall be held responsible for any injuries/damage, which may be caused as a result of the Contractor s failure to adhere to these requirements. 8.0 SERVICE DELIVERY SUMMARY: The Contractor service requirements are summarized into per formance objectives that relate directly to the mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success. B.4 QUALITY ASSURANCE SURVEILLANCE PLAN (QASP) This QASP was developed to evaluate contractor actions while implementing this PWS. It is designed to pro vide an effective method of monitoring contractor performance for each listed objective in the PWS. The QASP provides a systematic method to evaluate the services the contractor is required to furnish. This QASP is based on the premise that the Government desires to maintain a quality standard in the operation of the systems to be serviced under this contract, and providing the service is the best means of achieving that. The Contractor, not the Government, is responsible for management and quality control to meet the terms of the contract. The role of the Government is quality assurance, to ensure contract standards are achieved. The Contractor shall perform all work required in a satisfactory manner in accordance with the appropriate PWS paragraph above. The QA shall not consider the services complete until all deficiencies have been corrected. 1.2 PROCEDURES: The COR will inspect all performance objectives to ensure Contractor compli ance with the PWS. If inspection indicates unacceptable perfor mance, notify the Contractor of the deficiencies. Contractor shall be given adequate time to correct the unacceptable performance. If deficiencies are not corrected timely, the COR should notify the Contracting Officer for action.
