PROJECT INFORMATION: PROJECT NO. 520-16-102 REPAIR EMERMENCY DEPARTMENT FLOOR, BLDG. 3 VA GULF COAST VETERANS HEALTH CARE SYSTEM A/E SCOPE OF WORK Provide Professional Architect/Engineer (A/E) Services to include the preliminary services and investigative work and study providing a written report of findings and recommended course of action(s) to resolve the floor settling in the Emergency Department Building, Biloxi VA. Work of the A/E also includes the necessary planning, consultations and site visits, design work, preparation of contract drawings, construction details and specifications, the preparation of bid items, and if necessary, respond to contractor s and subcontractor s inquiries during the bid period, preparation of as-builts drawings, preparation of cost estimates, and construction period services as necessary for construction administration and installation of all work for Project No. 520-16-102. There will be three (3) submissions for working drawings and specifications; 25%, 75%, and 100% design submissions. All plans and elevation drawings shall be a minimum scale of 1/8 =1 0, large scale details (minimum ¼ =1 0 ) at largest possible scale to convey conditions of the work to be encountered. All site drawings shall be a minimum of 1 =30 scale. Drawings shall be saved to AutoCad Software Release 2017 and shall be stored on compact disk (disk to be submitted with design submissions mentioned above.) All drawings are to remain the property of the V.A. This project consists of a complete and thorough investigation into the sinking floor issue at the Emergency Department of Building 3, Biloxi VA and preparing contract documents to make necessary repairs and perform necessary work to ensure no further settling of floor. After a complete investigation as to cause(s) of floor issues prepare necessary contract documents to make necessary repairs and other required work to ensure VA that no further settling or cracking of the floor slab will occurr. Included in contract documents will be any necessary phasing plans for areas of the Emergency Department that may be disruppted during the construction period. It will be incumbent upon the AE to lead the design effort and to coordinate with the Contracting Officer Representative for site meetings and staff coordination of this effort. The existing Emergency Department building was constructed in 2007. The foundation system consists of pile supported spread footings. The four inch thick floor slab is reinforced with wire mesh on a compacted structural fill. The floor area of the Emergency Department is approximately 14,000 square feet. A geotechnical report is available for review that was made for this addition to the main hospital building. The concrete slab is not tied into the pile caps or perimeter building footing. The slab began settling not long after the sheet pile work was installed for the construction of the adjacent Building 30 Clinical Addition in 2008 and 2009. Several attempts have been made to raise the floor with injecting expandable foam under the slab. Both efforts did raise the floor but after time the settlement of the concrete slab re-occurred. There are void areas that have developed near the southeast area of the Emergency Department (ED) beneath the slab. The floor that has settled is also located in the southeast portion of the ED and consists of an approximate area of 3,000 square feet. It is sought to correct the settling floor areas while maintaining operation of the ED with the use of temporary stud walls and gypsum board to control dust and work flow. The project design and construction management shall be completed in accordance with this scope of work and all appropriate Department of Veterans Affairs (VA) Standards, specifications, and other criteria unless otherwise approved by VA. The design shall be reviewed and approved by a registered Architect and/or Engineer of the State of Mississippi. The following references, as a minimum, shall be utilized for planning and design purposes: H-08-1, VA Master Construction Specifications H-08-3, VA Construction Standards H-08-4, VA Standard Details NFPA, EPA, OSHA, and all applicable local, state, and Federal regulations VA TIL (Technical Information Library) First Design submission (25%): After site assessment and submission of written study with report of findings and recommended course of action(s) to resolve the floor settling; A/E shall make a formal presentation in person to discuss with VA Engineering Service the proposed project and estimates of construction.
