- GENERAL The Cleveland Veterans Affairs Medical Center (VA) anticipates award of an Indefinite Delivery Indefinite Quantity Contract (IDIQ), based on the contents of this solicitation. 1.1 SERVICE REQUIRED Intensive Outpatient Program, City of Canton, which is located in Stark County, Ohio Intensive Outpatient Program (IOP) is a six (6) bed (on any given day) twenty-four (24) hour/ seven (7) days a week community-based temporary residential housing program designed for Homeless or at risk for homeless Veterans who are struggling with problems associated with addictions, substance use and abuse, or the co-occurring disorders associated with substance use and mental health challenges. IOP bed program offers supportive housing and services for hard to reach, hard-to engage, Veterans at risk for homelessness, and dual diagnosed homeless Veterans who are challenged with a substance use disorder along with a co-occurring mental health diagnosis. Contractor shall provide housing, with the guaranteed minimum award amount for this contract of one veteran bed day of service 365 days (1 bed x 365 days) and the maximum aggregate value of orders that can be placed under this contract is 2,190 bed days (6 x 365 days) per year. The Government does not guarantee that it will place any order under this contact more than the guaranteed minimum award amount. The IOP provides private accommodations and allows for 24-hour/7 days per week eight (8) weeks residence (occurrence) for homeless Veterans. IOP Program length of stay is contingent upon the IOP treatment team recommendation of a standard 8-week residential admission. However, Veteran reserves the right to choose to leave IOP Programming at any time therefore bed days cannot guarantee a full 8 week stay per admission. It is understood that Veteran may be provided facility care at the contracted amount for a period not to exceed that of the stated length of treatment plan contracted generally 60 days. The Cleveland VAMC will place a new order or modify an existing order so that a Veteran may stay for longer than the original order. The IOP Treatment Team may recommend a longer length of stay than the original 8- week program. 1.2 BACKGROUND Ending homelessness among Veterans by 2015 is a national priority. The goal and plan for achieving it were introduced by the Department of Veterans Affairs Secretary in November 2009 at the National Summit on Ending Veterans Homelessness. In June 2010, the U.S. Interagency Council on Homelessness (USICH) released Opening Doors: Federal Strategic Plan to Prevent and End Homelessness, which is fully aligned with this goal. The Intensive Outpatient Residential Substance Use Disorder program is evidence based practice to help dual diagnosed homeless Veterans attain safe and stable housing. Cleveland VAMC HCHV Program, is an essential and critical part of the VHA, and is vital for providing a gateway to VA and community supportive services for eligible Veterans who are homeless. The HCHV Contract Residential Program for homeless Veterans is a vital program focused on addressing the needs of the most vulnerable and at-risk homeless Veterans. Ensuring that Veterans can be placed in community-based programs which provide quality housing and services that meet the needs of this special population is a particular focus of the HCHV program. Recent national trends in service provisions for the homeless outside of the Cleveland VAMC system include the utilization of contracted residential care in combination with Veterans Addictions and Recovery Center (VARC) as an alternative to shelter care or as a placement post unsuccessful discharge from an inpatient or residential treatment program due to relapse-related issues. Intensive Outpatient Programs for substance abuse disorders are a form of supportive housing that serves hard-to-reach homeless persons with severe mental illnesses and/or substance use disorders. The Cleveland VAMC provides diagnostic and therapeutic services in medicine, surgery, rehabilitation medicine, psychiatry, and skilled nursing home care. This facility also provides homeless services to those Veterans requiring a transition program to aid homeless veterans. 1.3 PLACE OF PERFORMANCE: Canton, OH (Stark County) 1.4 Additional Information: The facilities must be stand-alone, not mobile units. The agency must be able to begin housing homeless veterans immediately upon award.
