All information received in response to this RFI that is marked as proprietary will be handled accordingly. Responses to this notice are not offers and cannot be accepted by the Government to form a binding contract. Responders are solely responsible for all expenses associated with responding to this RFI. The submission of pricing, capabilities for planning purposes, and other market information is highly encouraged and allowed under this RFI In Accordance With (IAW) FAR Part 15.201(e). SOURCES SOUGHT DESCRIPTION This is NOT a solicitation announcement. This is a RFI/sources sought only. The purpose of this sources sought/RFI is to gain knowledge of potential qualified sources and their size classification relative to NAICS 339112 (size standard of 1000 Employees). Responses to this sources sought will be used by the Government to make appropriate acquisition decisions. After review of the responses to this sources sought, a solicitation announcement may be published. Responses to this sources sought synopsis are not considered adequate responses for a solicitation announcement. The Department of Veterans Affairs (VA), VISN 21 Network Contracting Office, is seeking sources that can provide the following, or brand name equal: Brand Name Info: Steris Steam Sterilizer Item Equipment # Description Quantity List Price 1.0000 SR2321211011 Amsco 400 26X37.5X60 Prevacuum Single Hinge Recessed, 1- Wall Rh Steam 208V/240V The AMSCO 400 SERIES 60 Medium Steam Sterilizer features a high capacity chamber that has been validated to sterilize up to 15 25lb instrument trays. 12 programmable cycles include Prevac 4+1, Gravity and Liquids. Cycle names are customizable. The chamber is constructed of solid 316L stainless steel and carries a 15 year warranty. The pressure vessel has a full jacket design to minimize the formation of excess condensation on the chamber interior. Steam activated gasket requires no lubrication and is warranted for 2 years. The unique door design features a patented single-hand, low-effort handle that makes operating a breeze. The enhanced display is located at an ergonomic height and features a large cycle countdown visible from 15 feet away. Ink on paper impact printer provides permanent cycle record. Utilities usage is minimized with multiple temperature sensors. System operation can be continuously monitored with vendor s remote service monitoring. Customer is responsible for decontamination and release of the equipment as non-hazardous. Equipment disposition from the staging point will be dependent upon Customer selection of disposal option. If removed equipment is to become vendor s property, it will be held on-site at the designated location for pickup by a vendor representative within a reasonable time, not to exceed five (5) working days. 1 1.0600 SE6045100071 Disposal of Sterilizer Vendor personnel will determine the most efficient method of disposal including but not inclusive of pick up by vendor s carrier or removal by local scrapping entity. If deinstallation is not performed by vendor personnel, Customer will decontaminate and move equipment to loading dock for pick up. Please note, disposal of Customer equipment implies Customer relinquishes ownership rights of disposed equipment. 1 1.0700 SE601852 Install - Medium Sterilizer Installation includes all necessary labor (non-union) and materials required to uncrate, set-in-place and assemble vendor s equipment. Price is for normal working hours, Monday Friday 8 am -5 pm unless otherwise noted. Vendor shall also include with its Installation Services an operational check-out test, any necessary adjustments and a demonstration of the equipment for appropriate facility personnel. 1 1.0800 SE010011 1st Year PM Evolution Medium Sterilizer Vendor 1st Year Maintenance Service is a preventative maintenance inspection program that is designed to keep new vendor equipment operating according to manufacturers requirements throughout the warranty period. Inspections are conducted using the appropriate vendor Engineering Service Preventive Maintenance Checklist and include any PM parts, lubricants, testing and adjustments necessary. Expendable and consumable parts are not covered in this program. 1 1.0900 SE160252 Install Proconnect (Per Unit) Install ProConnect (Per Unit). Remote Monitoring by vendor. Technical Support Services is included as part of this equipment purchase throughout the warranty period. This includes live remote monitoring and priority response for alarms of enabled vendor products. At the end of the warranty period, these services will be disabled unless a subscription is purchased or the equipment is added to Comprehensive Agreement. Note: The appropriate vendor specific kit must be ordered/installed by the responsible vendor Field Service Representative. 1 The information identified above and in the attached draft SOW is intended to be descriptive, not restrictive and to indicate the quality of the supplies/services that will be satisfactory. It is the responsibility of the interested source to demonstrate to the government that the interested parties can provide the supplies/services that fulfill the required specifications, or brand name equal. If you are interested, and are capable of providing the sought out supplies/services, please provide the requested information as well as the information indicated below. Response to this notice should include company name, address, point of contact, size of business pursuant to the following questions: Please indicate the size status and representations of your business, such as but not limited to: Service Disabled Veteran Owned Small Business (SDVOSB), Veteran Owned Small Business (VOSB), Hubzone, Woman Owned Small Business (WOSB), Large Business, etc.)? Is your company considered small under the NAICS code identified under this RFI? Are you the manufacturer, distributor, or an equivalent solution to the items being referenced above? If you re a small business and you are an authorized distributor/reseller for the items identified above or an equivalent solution, do you alter; assemble; modify; the items requested in any way? If you do, state how and what is altered; assembled; modified? Does your company have an FSS contract with GSA, VA NAC, NASA SEWP, or any other federal contract, that can be utilized to procure the requirement listed above? If so, please provide the contract number. Please provide general pricing of your products/solution for market research purposes. Catalog list prices, FSS prices, or any other publicized price lists are acceptable. Please submit your capabilities that show clear, compelling, and convincing evidence that you can meet the requirement described in the attached SOW. Please provide your DUNS number.
