- (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The requisition number is W912L8-18-Q-1002. The solicitation is issued as a Request for Quote (RFQ). (iii) This solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-73. (iv) The corresponding NAICS code is 325412 and the small business size standard in number of employees is 1,250. This action is deemed full and open. (v) List of contract line item number(s), items/description, and quantities - 2018-2019 Fluarix Quadrivalent Influenza Virus Vaccine in Prefilled Syringes (PFS), 10 PFS per package quantity: 180 packages (vi) Description of Requirements - Reference above (v) (vii) The ship to address will be the West Virginia Army National Guard AMEDD Medical Detachment at 100 Military Drive Charleston, WV 25309. Evaluation will be based on lowest price technically acceptable. (viii) The provision at FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition. (ix) The provision at FAR 52.212-2, Evaluation - Commercial Items, applies to this acquisition. The Government will award a contract resulting from this synopsis/solicitation to the responsible Quoter whose offer conforms to the solicitation and will be the most advantageous to the Government, price and other factors considered. (x) Quoters are to include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items, with its offer or acknowledge its listing in the System of Award Management (SAM). (xi) The clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition.
