Page 1 of Page 12 of 12 Page 1 of The Great Lakes Acquisition Center (GLAC) intends to negotiate a sole source contract with Johnson Controls for security upgrades to the existing security system located throughout the Clement J. Zablocki VA Medical Center and other clinics as identified in the Statement of Work (SOW). Johnson Controls is the only vendor capable of providing upgrades that will integrate with the P2000 security hub and within the current maintenance plan. This procurement is being conducted in accordance with FAR 12 & 13 Single Source Award Justification for Simplified Acquisitions Procedure and no other vendor will satisfy agency requirements. This notice of intent is not a request for competitive quotes. No solicitation documents are available and telephone requests will not be accepted. However, any firm that believes it can meet these requirements may give written notification to the Contracting Officer by 10:00 AM (CT) June 22, 2018. Supporting evidence must be furnished in sufficient detail to demonstrate the ability to comply with the above requirements. Information must be sent to the Contracting Officer, Lori.Eastmead@va.gov. TELEPHONE INQUIRIES WILL NOT BE ACCEPTED. Responses received will be evaluated however; a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, the Contracting Office will proceed with the sole source negotiation with Johnson Controls. SPECIFICATIONS/DESCRIPTION/WORK STATEMENT (VAMC SECURITY SYSTEMS EXPANSION PROJECTS) BACKGROUND The Veteran Affairs Medical Center, located in Milwaukee, Wisconsin to include the Community Based Out-Patient Clinics in Green Bay, Wisconsin, Appleton, Wisconsin, Union Grove, Wisconsin, Cleveland, Wisconsin, The CRRC (Community Referral and Resource Center) VA leased space, and the Great Lakes Acquisition Center and Human Resource leased space are equipped with high level security systems. The security systems consist of but not limited to, Intrusion alarms, wired and wireless panic alarms, Intrusion Detection systems, multiple camera and recording devices, phone and intercom systems, personal identification verification systems and physical access control systems. The Milwaukee Veteran Affairs Police Service currently has a service contract with a security company that provides preventive maintenance, emergency services, and limited upgrades to the current security features. Due to the certain levels of security requirements, to include continuous vulnerability assessment to each location, expansion and other upgrades are detrimental to the entire security network system and our facility is expected to provide security to control the integrity of our environment. STATEMENT OF WORK The contractor shall provide management, supervision, proposals, labor, equipment, supplies, installations, and final completion for all expansion projects requested by the Contracting Officer Representative or designee. All work with the proper approval for the building security systems will include Green Bay, Appleton, Cleveland, Union Grove Out-Patient Clinics; Community Referral Resource Center leased by the VA; Honey Creek Corporate Building leased by the VA and the Veteran Affairs Milwaukee Medical Center. All expansion projects must have required certifications for the security systems components and OSHA requirements. This is a statement of work for security systems and is for the VAMC, Green Bay Clinic, Appleton Clinic, Cleveland Clinic, Union Grove Clinic, CRRC, GLAC, and Human Resource Honey Creek and must provide proposal for services, programming, upgrades, licensing, to all approved proposals for security system expansion projects to include as-built when the project is completed. All statements of work for the expansion projects below will have the requirements needed to complete each project. REQUIREMENTS Line Item 1: Milwaukee VAMC Security Deficiencies Contractor will provide and install Electronic Physical Access Control Systems (PACS), for monitoring and controlling access to areas identified as requiring high or medium levels of assurance. Contractor will install Physical access control which will be required to meet all PIV-FIPS Complaints for the VA PACS System. Contractor will make every attempt to use pre-exiting Johnson Controls Panel, CK System, RDR s and any additional equipment necessary for the PACS system to be functional and to include all proper programming and mapping. Contractor will include proper door contacts with the PACS System. Contractor will install the PACS to the following locations: Canteen Storage, Building 111, lower back door, room B0012R. Building #111, Room 10528 Secondary access point from room 10531. Two (2) High Assurance PACS installation - Building #20, 3rd Floor stairwell and roll up access point (IT Long Term Storage). IRM Storage area, Building 70, room 70-D-33. Employee Health Records Building 70, room 70-E-46. Contractor will also install Motion Intrusion Detection in Building #70, room #70-E-46 (Veterans Records Storage). The intrusion detection alarm system will detect entry into the room and broadcast a local alarm of 80 to 90 db within the protected area. This intrusion alarm notification must also be tied in to the P2000 system for notification at Police Operations and allow for a reset capability. This alarm notification will be programmed with a pop up alert to the P2000 system stating; Motion Intrusion, 70-E-46, Police Response Required. Contractor will ensure all proper programming, mapping and testing of the installed system. Line Item 2: CBOC Security Deficiencies Contractor will provide and install one (1) fixed camera in the Green Bay CBOC Canteen Storage room (1K006) behind the food counter. This camera will have low light capability and the ability to be viewed at VA Police Operations in Green Bay Dispatch, with a minimum 60-day record capability. The fixed camera must also be tied into the P-2000 and motion alarm capabilities with a Pop up alarm. The camera will need one (1) Milestone corporate license. The camera will be installed onto the milestone server/management server, and licensed under the corporate level for police services. Contractor will provide all programing, mapping and proper recording for the device to include the basic requirements of 15 frames per second. Contractor will provide and install four (4) total fixed cameras in the Appleton CBOC Pharmacy main area (168A) and narcotics room (168B). The cameras will have infrared capability and the ability to be viewed at VA Police Operations in Green Bay Dispatch, with a minimum 60-day record capability. The fixed cameras must also be tied into the P-2000 and motion alarm capabilities with a Pop up alarm. The cameras will need four (4) Milestone corporate licenses. The cameras will be installed onto the milestone server/management server, and licensed under the corporate level for police services. Contractor will provide all programing, mapping and proper recording for the devices to include the basic requirements of 15 frames per second. Contractor will provide and install one (1) hard wired panic alarms for Appleton CBOC Pharmacy Vault. Alarm activation lights will be mounted on the wall outside the room. This alarm will require pop up alerts through the P-2000 system and must have the capability to be monitored via P-2000 at the Green Bay CBOC Police Operations and contractor will provide all programing, mapping. Contractor will provide and install Electronic Physical Access Control Systems (PACS), for monitoring and controlling access to areas identified as requiring high or medium levels of assurance. Contractor will install Physical access control which will be required to meet all PIV-FIPS Complaints for the VA PACS System. Contractor will make every attempt to use pre-exiting Johnson Controls Panel, CK System, RDR s and any additional equipment necessary for the PACS system to be functional and to include all proper programming and mapping. Contractor will include proper door contacts with the PACS System. Contractor will provide and install a mortise style lock system with the PACS system and must meet the following. Mortise lock systems must allow for single motion egress. The installation of high security exit devices meeting NFPA Life Safety Code standards is appropriate. Doors set in steel frames must be fitted with a mortise lock with a deadlock feature. IAW ANSI/BHMA A156.13 American National Standards for Mortise Locks. The day lock on the main door must be automatically locking, with a minimum 19 mm (3/4 in.). Contractor will install the PACS to include mortise lock system to the following locations: Appleton Building 1 Warehouse Storage room 207. Contractor will only upgrade current PACS system to PIV Compliant PACS to the Pharmacy main entrance and secondary entrance, however upgrade door 168 to a mortise lock system and move the PACS system from 168A. Contractor will coordinate with Appleton CBOC building owner for installation of mortise style locks and provide opportunity for sub-contracting of choice. Line Item 3: Wander guard Tie-In Contractor will provide, install and tie in Building 111 9th floor elevator cameras for the Milestone system to work cooperatively with the Wander guard notification system. The contractor will program the system to alarm the milestone and P-2000 system when the Wander guard is activated for immediate reviewing for Police Dispatchers and proper recording. This will be done by a Pop up System displaying the activated camera on the P2000 monitor at Police Dispatch desk. Line item 4: Upgrade Cameras Contractor will provide and upgrade of ten (10) total PTZ cameras from analog to digital. The camera locations are as follows: Building 43 North exterior; South exterior; West exterior; smoke Shelter; Building 111 Police Parking Lot; Area # 1,2,3, &35; Ambulance Bay; Rooftop NE corner; Area #10; DVN Air/Gas Tank. The locations listed are under Milestone camera view descriptions. All cameras will have the ability to be viewed at VA Police Operations, with a minimum 60-day record capability and the basic requirements of 15 frames per second. Line Item 5: P2000/Milestone install Contractor will provide and install one computer with monitors in the Physical Security office in building 111, room B0438, for the P-2000 security system and the Milestone system. This workstation will possess the same capabilities and functions of the P-2000 and Milestone workstations currently located in Police Operations. Contractor to install updated versions of P-2000 and update computer to maintain effective performance to the Dispatch Operations console area. Contractor will also install video cards and computer to meet all Police monitors to view the Milestone CCTV system. Contractor will install data lines as needed to ensure operability of the workstation. Line Item 6: Camera installation Contractor will provide and install fixed cameras to the Building #111 designated smoking shelters (two cameras), 2nd floor Operating Room pharmacy dispensing room 2224 (two cameras), Building #70 1st floor, basement hallway areas and southwest exterior. These cameras will have the ability to be viewed at VA Police Operations, with a minimum 60-day record capability. Cameras shall provide usable images in low-light conditions. Total of thirteen (13) cameras will be installed onto the milestone servers/management server, and licensed under the corporate level for police services. Thirteen (13) Milestone corporate licenses will be needed for the installation and use of the cameras. Contractor will provide all programing, mapping and proper recording to all devices. The installation areas as follows: Building #111 patient smoking shelter adjacent Spinal Cord Injury entrance; Building #111 employee smoking shelter adjacent Spinal Cord Injury entrance; Building #111 2nd floor Operating Room pharmacy dispensing room 2224; 70 D-wing main hallway intersection; 70 D-wing north end hallway facing Post Office; 70 C-wing main hallway intersection; 70 C-wing north end hallway adjacent Bio-Med; 70 B-wing main hallway intersection; 70 B-wing north end split facing Animal Research and loading dock; 70 E wing intersection; 70 1st floor Directors Hallway three-way split; 70 southwest exterior of building with visibility of the south and west areas. All cameras will have the ability to be viewed at VA Police Operations, with a minimum 60-day record capability and the basic requirements of 15 frames per second. Line Item 7: Fluid mesh installation Contractor will provide and install a Fluid mesh 1200 VOLO wireless link on emergency towers 9 and 10 for wireless interface with the CCTV system mounted on the Police observation tower. This wireless link will provide for the Police tower cameras to be viewable in Police Operations based on line of sight connections between the Police tower and the wireless links. Line Item 8: Physical Access Control/Panic alarm installations Contractor will provide and install Electronic Physical Access Control Systems (PACS), for monitoring and controlling access to: Building #111 rooms 1535, 5229, 6609 (two/2 - east and west exit), 2nd floor ICU west exits (double doors) adjacent 2508 and 2534, 5th floor Cath lab north exit (double doors) adjacent 5657; Building 70 C-219 (double doors), and 70-C-220; Building #43, room B104 (double doors). Contractor will move card reader from Building #111, room 1811 to room 1821; Building 5, room 206 with additional door contact to west exit door of same room. Contractor will include proper door contacts with the PACS System. Physical access control systems will be required to meet all PIV-FIPS Compliance for the VA PACS System. Contractor will make every attempt to use pre-exiting Johnson Controls Panel, CK System, RDR s and any additional equipment necessary for the PACS system to be functional and to include all proper programming and mapping. Contractor will also provide and install six (6) hard wired panic alarms to the following locations: Building #111 6300B; 10654; Building 5 1st floor, room 122; Building 70-E-1058; Building 123 rooms A-118 and B-142. These alarm points require a pop up notification and monitoring capability through the P2000 in Police operations. Contractor will make every effort to use existing cabling and provide for all programming. Contractor will also provide 25 personal protection alarm screechers with at least a 130 db alarm. Line item 9: Bosch workstation installation Contractor to provide and install (1) Bosch monitor system with (1) 24-inch monitor and audible sound in the Building #111, Ward 3CS Nurse s Station. Contractor will program and setup workstation to work with the Dispatch Bosch system and to include any software upgrades, programming and proper mapping in the Bosch program. Contractor will provide and include all cabling, UPS System, training and commissioning. Line item 10: Honey Creek lockdown Contractor will program and setup Honey Creek location with lockdown programming to work with the P-2000 system and to include any software upgrades, programming and proper mapping in the P-2000 program. Contractor will provide all programming and commissioning. Line item 11: Milestone Server Upgrade for Green Bay CBOC and provision of a Handheld Forward Looking InfraRed (FLIR) Unit Contractor will provide expansion to the Milestone Closed Circuit Television camera system at the Green Bay Community Based Outpatient Clinic. This server upgrade will provide for a minimum of 60-day record capability for the Milestone camera system at that facility. Contractor will also provide, program and setup to include orientation and operator training for 1 hand-held Forward Looking InfraRed FLIR camera systems with thermal energy detection capability. 1. CONTRACT EFFORT REQUIRED General: It is of utmost importance that the contractor utilizes adequately skilled and productive labor to furnish the required level of services specified in this contract. Failure on the part of the contractor to perform satisfactory work and obtain satisfactory results shall necessitate Government action to adjust the contractor s monthly invoices accordingly and/or pursue contract termination. Contractor s personnel are required to sign-in immediately upon arrival on-site, at the Police Operation Center (Building 111), and sign-out at time of departure. Contractor will coordinate all actions with the Project Manager with regards to any door lock or door modifications and installations. This guidance will also apply to any window modifications. 2. DEFINITIONS/ACRONYMS CO Contracting Officer COR Contracting Officer s Representative PIV- Personal Identity Verification PACS- Physical Access Control Systems FIPS 201- Federal Information Processing Standard VAMC Department of Veterans Affairs Medical Center to include Honey Creek and Appleton Normal Work Week Monday through Friday, 7:30 a.m. 4:30 p.m. excluding federal holidays. Competence/Competency A determination by the contractor that its personnel performing maintenance, repair, and inspection services have the requisite individual training, experience, certifications and capability to perform expected work. Environment of Care Program Consists of basic policies and procedures, VAMC specific, to be provided to the contractor to familiarize contractor s personnel with the work environment before the contractor begins on-site work activities. Purpose is to promote safe and effective on-site work performance. OEM- Original Equipment Manufacturer CRRC Community Referral and Resources Center Federal Holidays Observed by the VAMC: New Year s Day Labor Day Martin Luther King Day Columbus Day Presidents Day Veteran s Day Memorial Day Thanksgiving Day Independence Day Christmas Day 3. SERVICES REQUIRED As a minimum, inspections, preventive maintenance and all expansions shall be completed as required by and in accordance with the following: 1. National Fire Protection Association National Electrical Code, NFPA 70-latest edition 2. National Fire Protection Association Electrical Equipment Maintenance, NFPA 70B-latest edition 3. National Fire Protection Association Electrical Safety Requirements for Employee Workplaces, NFPA 70E-latest edition 4. National Fire Protection Association Health Care Facilities, NFPA 99-latest edition Expansion installations services shall be performed within the normal workweek. No non-emergency services shall be performed at other than the normal workweek unless prior approval of the COR (or designee) is obtained. All approved proposals will be coordinated with the Police to ensure all components are met for the installation. Acquisition and installation of software updates/upgrades. Perform database back-ups must be coordinated with the VA Police. Provide consultative services to conduct and document performance improvement reviews and resolve system operational problems not covered by other contracts. Security upgrades and expansions shall be completed upon approval unless the contractor requests and obtains approval to perform work at a later date and time. Requests shall be submitted, in writing, to the COR and state: - Reason for requested delay in performance of approved proposal work: - Equipment and functions affected by the delay; - Date and time project are requested to begin: - Duration of project; and - Operational status of equipment during performance of the upgrade. 4. MATERIALS AND PARTS TO BE FURNISHED, INSTALLED, AND/OR REPAIRED BY THE CONTRACTOR Solvents and lubricants shall be of the type specifically approved by the manufacturer of the equipment. Parts/components shall be approved for use by the manufacturer of the building security system to include approval from the COR or designee. Software and hardware updates/upgrades shall be approved for use by the manufacturer of the building security system to include approval from the COR or designee. 5. TEST EQUIPMENT Prior to commencement of work on this contract, the contractor shall provide the VAMC with a copy of the current calibration certification of all test equipment, which is to be used by the contractor on VAMC s equipment. After each proposal is completed by the contractor it must be tested and verified through the Physical Security Specialist. Test equipment calibration shall be traceable to a national standard. 6. UTILITIES FURNISHED BY THE GOVERNMENT Electrical power at existing outlets for the contractor to operate such equipment as is necessary for the performance of work required by this contract. 7. SERVICE MANUALS/TOOLS/EQUIPMENT The VAMC shall not provide tools, (test) equipment, service manuals or service diagnostic software to the contractor. The contractor shall obtain, have on file, and make available to its service personnel all operational and technical documentation (such as operational and service manuals, schematics, and parts list) which are necessary to meet the performance requirements of this contract. The location and listing of the service data manuals, by name and/or the manuals themselves shall be provided to the CO upon request. 8. FACILITY BUILDING SECURITY SYSTEMS COMPONENTS INCLUDED WITHIN THE SCOPE OF THE CONTRACT All items noted above are the facility building systems components and are within the scope of work, unless otherwise noted. Contractor must be able to access systems remotely. Contractor must rapidly resolve problems to minimize the impact of safety and business operations. Contractor must be able to provide a service team that is certified and has knowledge of all system and component needs. Contractor must provide written reports/assessments for testing on all expansions and status of normal status of any upgrade/expansions and as-builts. Contractor shall advise the VA of changes in code requirements, reliability standards, and performance improvements as recommended by the manufacturer(s). 9. SERVICE NOT COVERED This contract does not include the performance of any work, as determined solely by the VA, required because of negligence, improper use, accident, vandalism, power line fluctuations, or damage by work performed by others for which the contractor is not directly responsible. The contractor shall not supply nor install new attachments as may be recommended or directed by inspection firms or by State, Municipal, or other Federal Government authorities, unless approved in writing by the Contracting Officer or designee. 10. COMPLIANCE WITH SECURITY REQUIREMENTS IDENTIFICATION, PARKING, SMOKING, AND VA REGULATIONS: The Contractor's employees and/or subcontractors approved by the COR, shall wear visible identification at all times while on the premises of the VAMC. Identification shall include but not be limited to a company identification card, which shall include the company name, employee name, title, and a photograph of the employee. All employees of the Contractor or Subcontractor providing services to the Government must wear a company uniform identified with the company name and/or logo. Uniforms must be neat and clean at all times while working the VA premises and will be approved by the COR. Contractor s personnel may be required to possess and wear VA issued identification cards in addition to the above requirements. It is the responsibility of the Contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police Section. The VAMC will not invalidate or make reimbursement for parking violations of the Contractor under any conditions. Smoking is prohibited inside any buildings at the VAMC. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court. The Contractor will be provided keys to access areas in the building. The Contractor shall be responsible for any lost keys, card keys. Associated hardware and any inherent damages (i.e., re-keying of whole building). This cost shall be withheld from payment(s). The decision to re-key the whole facility is solely that of the Government. Any interior or exterior doors that must be unlocked to gain access shall be kept locked during the performance of work. Should any other devices need to be unlocked like an entry gate, those devices must be locked immediately upon entry or exit. At no time shall the contractor allow any person into a locked area or loan keys/access cards to another person. Contractor may be held in default for failure to comply with this requirement and subject the Contractor and employees to contract and other legal remedies. Reserved. VA Contact Person: The Contractor agrees that none of its officers or employees shall use or reveal any research, statistical, medical, patient specific, or security information obtained in the performance of the work without the written consent of the Government. 11. COMPLIANCE WITH THE ENVIRONMENT OF CARE PROGRAM (SAFETY REGULATIONS) In performance of this contract, the Contractor shall train its employees to follow the basic VAMC Environment of Care policies/procedures and standard safe work practices, and takes such safety precautions as the Safety Manager or his designee may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contracting Officer or his/her designee will notify the Contractor of any noncompliance, and the Contractor shall immediately correct the conditions to which attention has been directed. Such notice, when served on the Contractor or his representative at the site of the work, shall be deemed sufficient for the purposes aforesaid. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue and order halting of all or any part of the work, and Contractor may be held in default. Contractor shall provide to the Contracting Officer, prior to contract start date, Material Safety Data Sheets (MSDS) for all potentially hazardous materials (lubricants, cleaners, working fluids, etc.) to be used in the facility in performance of the contract, and will not use, in the facility, such materials which have not been so cleared in advance with the VAMC. MSDS for new chemicals shall be furnished concurrently with arrival of the chemicals on site. Additionally, a copy of all required MSDS shall be maintained at the chemical storage site in a location, readily accessible to VA personnel. NOTE: OSHA requires used rags and flammable materials to be stored in fireproof containers (provided by the contractor). 12. INVOICES AND PAYMENT Invoices will be submitted when completion of proposal work is done in arrears to the Government Office designated in the solicitation and include the original and one copy. To constitute a proper invoice, it must include, as a minimum the following information: Name of the business concern and invoice date. Contract and purchase order numbers or other authorization for rendering services. Failure to cite these numbers may cause a delay in processing payment. Description, price, and quantity of property and services actually delivered or rendered. Name (where practicable), title, phone number, and complete mailing address of responsible official to whom payment is to be sent and any other substantiating documentation or information as required by contract. 13. INSURANCE Worker compensation and employer s liability. Contractors are required to comply with applicable Federal and State Worker Compensation and occupational disease statutes. General Liability. Contractors are required to have Bodily Injury Liability Insurance coverage written on the comprehensive form of policy of at least $1,000,000 per occurrence. Property Damage Liability. Contractors are required to have Property Damage Liability insurance coverage of at least $1,000,000. 14. UNAUTHORIZED USE OF GOVERNMENT EQUIPMENT The Contractor shall not allow his/her employees, including Subcontractors employees, to open desk drawers, cabinets, or to use office equipment, including the use of non-pay telephones for any purpose other than a local emergency call. 15. TAX EXEMPTION The Federal Government is exempt from most taxes. Exemption Certificates indicating the Medical Center s tax-exempt status will be furnished by the Contracting Officer upon request.
