Description
1 DRAFT *STATEMENT OF WORK (SOW)*DRAFT MEDICAL GAS TESTING CONTRACT NOTE: No security accreditation package is required and Contractor will not have access to any VA sensitive information. This project does not require acquisition of a service that involves storage, transmitting or exchanging VA sensitive information as this requirement is strictly a construction activity per the SOW below. PROJECT DESCRIPTION: To issue a (1) one year service contract with (4) one year options to provide Annual Medical Gas testing and certification, and Annual Master Alarm testing at the Phoenix VA Medical Center 650 East Indian School Road, Phoenix Arizona. Testing and Certification of the Medical Center s Medical Gas systems shall comply with Engineering Policy Memorandum No. FS/138-1027. This policy shall be used to schedule, monitor and document the testing and maintenance of the Medical Center s Medical Gas systems. These systems include Oxygen, Medical Air, Vacuum, Waste Anesthesia and Nitrous Oxide. All critical components of the piped Medical Gas system including Bulk Oxygen tanks, Medical Vacuum systems, Medical Air systems, Waste Anesthesia Vacuum system, Dental Air, Dental Vacuum, Oxygen H tank supply manifolds, Nitrous H tank supply manifolds, master alarm signal panels, area alarms, automatic pressure switches, shutoff valves, flexible connectors and outlets shall be tested, inspected and maintained in compliance with NFPA standards by the contractor. The Prime Contractor shall have completed NFPA 99C Medical Gas Certifier training and provide current certificates of qualification. Tasks: The Contractor shall provide Annual Medical Gas testing and certification of 100% of Oxygen, Medical Air, Vacuum, Waste Anesthesia and Nitrous outlets at the Phoenix VA Medical Center (Building 1), the Community Living Center (Building 16), the Ambulatory Care Center (Building 8) and Dental Clinic (Building 29). The Contractor on the initial Inspection and Certification visit, shall complete an accurate inventory of Medical Gas outlets of all Buildings covered by the contract and provide a current Medical Gas Campus Overview book within 30 days of the initial site visit. The Contractor shall check flow rate at each outlet. Oxygen, Medical Air and Nitrous Oxide must deliver a minimum of 100 liters per minute (1PM). Surgical outlets must deliver a minimum of 142 lpm. Medical Vacuum outlets must provide a minimum of 85 lpm. The Contractor shall test each outlet (except Medical Vacuum) for suspended particulate by passing a gas stream through a particulate filter at a maximum flow rate for 5 seconds. Any presence of discoloration or visible particulate matter shall be documented. The Contractor shall test each Oxygen, Medical Air and Nitrous Oxide outlet for purity using a certified and documented meter. Oxygen and Nitrous Oxide shall be delivered at a purity level greater that 99.0%. The Contractor shall check the static pressure at each outlet. Oxygen, Medical Air and Nitrous Oxide must have static pressure value of between 50 and 55 psi. Medical Vacuum shall be a minimum of 12 of Hg. The Contractor shall ensure each Medical Air outlet conforms to grade N standards. The Contractor shall use certified and documented equipment to check Medical Air for carbon dioxide, sulfur, nitrous fumes, carbon monoxide, oil concentration and dew point. All levels shall meet the criteria defined by CGA G7.1ANSI grade N air. The Contractor shall annually inspect and test for leaks, all nonstationary booms and articulating assemblies, other than head walls utilizing flexible connectors per manufactures recommendations. The system pressure to nonstationary booms and articulating arms shall be maintained at operating pressure until each joint has been examined for leakage by effective means of leak detection that is safe for use with Oxygen. The Contractor shall check each outlet for leaks and visible damage. The Contractor shall provide Annual Medical Alarm Panel testing for proper operation at (3) three locations, H117, the Graphic Room in the Boiler Plant, H107 the Maintenance Supervisor office and E100A the Operators room at the Phoenix VA Medical Center. The Contractor shall verify set points at the Oxygen bulk tanks. The Contractor shall inspect zone, riser, source and main lines valves for proper labeling, locking devices and visible damage annually. The Contractor shall annually calibrate carbon dioxide and dew point meters and provide documentation of the calibration. The Contractor shall ensure each Medical Air compressor conforms to grade N standards. The Contractor shall use certified and documented equipment to check Medical Air for carbon dioxide, sulfur, nitrous fumes, carbon monoxide, oil concentration and dew point. All levels shall meet the criteria defined by CGA G7.1ANSI grade N air. The Contractor shall provide an Annual updated VA Medical Center Medical Gas Overview Book. The Contractor shall provide documentation for certified and trained staff in the testing and certification of Medical Gas testing and Certification of Medical Gas Outlets certificates shall be presented to the Contracting Officer Representative prior to commencement of testing and certification. The Contractor shall provide an electronic report within 10 working days to the Contracting Officer, the Maintenance Shop Supervisor and the Maintenance and Operations Supervisor The Contractor shall provide electronically a detailed discrepancy list with explanations and a proposed correction to each discrepancy. The Contractor shall respond to requests for emergency services in the following manner: A competent service representative shall respond and arrive at the Phoenix VA Medical Center facility within two (2) hours from the time of request for service. All requests for service shall be considered emergency requests unless a statement to the contrary from the Engineering representative. The Contractor shall restore the facility to full performance within 24 hours of the originating request for service. GENERAL NOTES: The Contractor shall provide notification one (1) week prior to site visit to the Engineering authority. Testing and certification shall be accomplished by certified inspectors. Certificates shall be provided prior to the inspection process. The contractor shall provide inspection, testing and certification cost for one (1) year and four (4) additional optional years at the Phoenix VA Medical Center. The C&A requirements do not apply, a security Accreditation Package is not required. The Contract does not involve the storage, generating transmitting or exchanging of VA sensitive information. No sensitive information will be handled by the Contractor. No hazardous materials are expected to be encountered during this contract. If the presence of hazardous materials is suspected, the Contractor shall notify the Engineering representative immediately. Important information: The Government is not obligated to nor will it pay for or reimburse any costs associated with responding to this Sources Sought Notice request. This notice shall not be construed as a commitment by the Government to issue a solicitation or ultimately award a contract, nor does it restrict the Government to a particular acquisition approach. The Government will in no way be bound to this information if any solicitation is issued. The VA is mandated by Public Law 109-461 to consider a total set-aside for Service Disabled Veteran Owned Small Business set aside. However, if response by Service Disabled Veteran Owned Small Business firms proves inadequate, an alternate set-aside or full and open competition may be determined. No sub-contracting opportunity is anticipated. The North American Classification System (NAICS) code for this acquisition is 541990 ($15.0M). Notice to potential offerors: All offerors who provide goods or services to the United States Federal Government must be registered in the System for Award Management (SAM) at www.sam.gov and complete Online Representations and Certifications Application (ORCA).
