- Synopsis: The Department of Veterans Affairs herein referred as the VA has conducted oral solicitations under the authority of FAR 13.106-1(c), for the procurement of pathogen free ice machine cleaning and maintenance of 21 Ice Machines for the Connecticut Health Care System, West Haven Campus located at 950 Campbell Avenue West Haven 06516. The cleaning may be completed up to 4 times per year based on the need of the facility and per determination by the COR. The anticipated contract will be a Firm Fixed Price (FFP) Commercial Purchase Order and will include (4) one year option periods. The anticipated award date is July 9, 2018. This procurement has followed the procedures of oral solicitations. The applicable NACIS code is 811310. The size standard as defined by the U.S. Small Business Administration is $7.5 million. All responsible sources may submit a capability statement, proposal or quotation which shall be consider by the Department of Veterans Affairs and received prior to July 2nd at noon eastern standard time. All information submitted should support the offeror's capability to provide the items required, and shall be furnished at no cost or obligation to the Government. Responses must be in writing and must provide clear and concise documentation indicating an offeror's capability to provide these services. Verbal responses are not acceptable and will not be considered. Questions shall be submitted electronically to Lorrie Baines. Lorrie.baines@va.gov Statement of Work Ice Machine Maintenance VACHS WEST HAVEN CAMPUS FACILITY MANAGEMENT SYSTEM/ Statement of Work Due to the Urgency of this Service/ Repair the contractor will commence work immediately upon the Award of this Purchase with coordination of the CO/COR at the VAMC. -The Contractor will be required to supply pull hot work permits as needed. -The Contractor may be required to work outside of normal business hours as needed in a case to case basis. -The Contractor will report daily of progress of job and follow all required VHA Policies and Directives The Vendor will provide the following services: Scale removal Sanitization of reservoir Filter cleaning or change Clean condensers Sanitization of Ice Bin Flush of water dispenser line Reports with pictures before and after cleaning of each ice machine. Certificate of cleanings were completed. The following Ice Machines will be serviced: Building 1 G-186-A Cat Scan 3 West 4 East 4West 5 East 5 South Step Down Swing Space 5 West Step Down 6 East Nursing 8 East Psych Ward APU 3rd floor west side nourishment center (Big Room) Building 2 ER Psych ER 4104 Eye Clinic 4123 Coumadin Clinic 5127 Cancer Center 6201 Blind Center 7160 Research Lab Dialysis suite Outer Buildings: 12A 15A Building 21 Building 22 Intent and Requirements: A. General Requirements & Considerations Statements: 1) Capability Requirements: The Contractor Service Representative(s) who perform services under this contract must be competent, experienced and qualified to perform such services listed herein. All work performed will be first class in accordance with good practices. The Contractor shall submit proof of appropriate training, equipment knowledge and experience for all personnel performing services of cleaning practices of Ice Machines. 2) Licenses and Permits: The Contractor shall, without additional cost to the VACHS, provide and maintain all licenses and permits for operational personnel, equipment, vehicles and other resources required for accomplishing all requirements per this contract specification in accordance with all applicable Federal, State, Municipal, and Local Regulations (e.g. OSHA, EPA, CT-DEEP). If appropriate licenses are not maintained in accordance with requirements, the VACHS may terminate the contract. 3) Regulatory Documentation The Contractor will be required to comply with changes to Federal, State and local regulations and procedures which occur during the term of this contract. The Contractor will provide to the VACHS any additional certifications that may be required because of changes in such laws. 4) Building Occupancy The VACHS will maintain full occupancy of the site for the duration of the services required under this contract. The Contractor shall not interfere or hinder the daily operations of the VACHS while performing services. 5) Stop Work Option: The VACHS reserves the right to halt work if the CO or COR determine work is being done in an unsafe manner. VACHS will not incur additional costs if work is halted for good cause. 6) Delivery and Performance Schedules: a) It is the intent of this contract to have ongoing and regular services along with emergency services as required. The responsible CO or COR shall notify the Contractor of the necessity for emergency services. The Contractor must coordinate all work on site through the facility s COR. Contractor s performance shall be within the Contract Working Hours except in the case of emergencies and or as listed in Notes. b) The VACHS requires services on a recurring basis and for needs not know at this time. On occasion, there may be a need for additional on-site services. If additional on-site services are needed, they will be charged and billed separately at prevailing rate. 7) Identification, Check-in, Parking, and Smoking Regulations: The Contractor s Service Personnel shall wear visible identification at all times while on VACHS premises. Contractor s Service Personnel will be required to sign in once on campus at the Building 15 Facility Management Service (FMS) Building or at the Building 16 (Boiler Plant) Office Area. Contractor s Service Personnel will always report to COR once on campus and before the start of any work. The Contractor s Service Personnel will park vehicles in appropriate designated parking areas. Information on parking is available from the Facilities Management Service (B15) or the VACHS Police Service. The VACHS will not invalidate or make reimbursement of parking violations of the Contractor s Service Personnel under any conditions. Smoking is prohibited inside any buildings at the VACHS. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VACHS regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court. 8) Invoices and Payment: The Contractor s payment of services rendered under this contract will be made in arrears and upon completion of each service on monthly bases. The Contractor shall submit a properly prepared invoice for payment to the address shown elsewhere herein or as provided by the CO. Invoices shall reference the Contract Number and Purchase Order Number, provide a complete and accurate description of services/ supplies delivered/ rendered, including dates of performance, amounts, unit prices (as applicable), extended totals and any other data relevant for payment purposes. The Government shall not authorize payment until such time that the COR receives all properly executed and signed documentation as required by CO. B. General Statements: 1. General Statement A) Repair & Replacement Services. The Contractor shall, with prior written approval of the CO or authorized designee (COR), repair or replace failed or worn components per this contract specification and as indicated. NOTE: ANY ADDITIONAL CHARGES CLAIMED FOR MATERIAL AND LABOR NOT COVERED BY THESE CONTRACT SPECIFICATIONS OR AS INDICATED IN THE EQUIPMENT LIST MUST BE APPROVED BY THE CO BEFORE SERVICE IS RENDERED!
