- Cable Pull and Accessories Installation Services VA Medical Center Sheridan WY 82801 BRIEF SCOPE OF WORK (This is for information only) Project Scope The contractor shall provide all labor, personnel, equipment and test equipment, tools, supervision, parts, materials (to include all cable and any needed accessories and/or other items) necessary to provide cable and accessories installation, onsite for Communications Systems located at the Sheridan VAMC facility and as defined in this Statement of Work (SOW). Purpose: Install cabling and terminations for expanding Network Infrastructure to support device (video) integration in various locations throughout the medical facility. Background: The facility is bringing on line, via the information network, an information display system that will provide veterans with health information and facility events relevant to educating veterans and improving their well-being. The video displays are located in patient waiting areas of which there are 6 that require a network connection. Network Cable Specifications: All cabling installed to be Plenum rated CAT6. Cabling is to be terminated (punched down) per ANSII standards. On the display end of the cable run, wall plates are to be mounted, allowing a male CAT6 plug to connect to the network and the color to be white, off-white, or eggshell. Wiring above the overhead ceiling (grid) will be suspended so as not to lay directly upon ceiling tiles. Utilizing existing cable trays are recommended. Prior to performing any new fire wall penetrations, the Safety Officer requires the proper documentation (provided by the Safety Office) and that the contractor follow all NFPA standards for ensuring fire wall integrity. On the network side (switch) the terminated and labeled cable will be run to the switch plus a 3-meter maintenance loop added at the network switch equipment rack. All completed cables are to be tested to ensure continuity. All services for this contract must meet manufacturer performance and technical specifications, Federal Regulations, and other specifications that may apply such as National Electric Code (NEC), Life Safety Code (LSC), Joint Commission (JC), and VA Regulations, American National Standards Institute (ANSI), Electronic Industries Alliance (EIA)/ Telecommunications Industry Association (TIA), and Building Industry Consulting Service International (BICSI) Telecommunications Distribution Methods Manual (TDMM). VA has adopted the National Fire Codes (NFC), except NFPA 5000, published by the National Fire Protection Association (NFPA). Scope of Work Locations: All cable runs connect a video device (patient/veteran waiting areas) to the nearest IT communications closet as follows: Dental Waiting Area -- 2nd Floor, Bldg. 71 -- 170 cable run (not including maintenance loop of 3 meters per switch, if required) Radiology Waiting Area -- 2nd Floor, Bldg. 71N -- 78 cable run (not including maintenance loop of 3 meters per switch, if required) Ambulatory Care Waiting Area -- 1st Floor, Bldg. 71 -- 75 cable run (not including maintenance loop of 3 meters per switch, if required) Pharmacy Waiting Area -- 2nd Floor, Bldg. 71N -- 38 cable run (not including maintenance loop of 3 meters per switch, if required) Laboratory Waiting Area -- 2nd Floor, Bldg. 71N -- 50 cable run (not including maintenance loop of 3 meters per switch, if required) Mental Health Waiting Area -- 1st Floor, Bldg. 8 -- 38 cable run (not including maintenance loop of 3 meters per switch, if required) SPECIAL WORK REQUIREMENTS: Hours: Services shall be performed according to specifications of Government point of contact and COR. The regular work hours four this facility is Monday through Friday 0730am to 4:00pm (MST). Special Instruction: The contractor's representative will schedule all work with the Engineering Department. All contractor personnel (without exception) must report to the Engineering Department, Bldg. 5-room 208, to sign in and receive a contractor identification badge before any work is performed and return to the same location to sign out and turn-in said badge along with the documented service report for the service performed. Contractor will pay $75.00 per badge (VA Property) not returned each day. Approval of installation will follow inspection parameters as stated in quality assurance surveillance plan (QASP). Noncontract Charges: The contractor shall not perform any service that will result in additional charges without prior approval from the Contracting Officer or COR. Property Damage: The contractor shall take all necessary precautions to prevent damage to any government property, and will notify the Contracting Officer immediately if damages occur. The Contracting Officer will authorize the contractor to remedy the situation in one of the following ways: be assessed current replacement costs for damaged property, replace damaged property in a timely manner, or correct the damages with like materials at no additional cost. Identification, Parking, Smoking, and VA Regulations: The contractor will need to acquire a badge for each worker, for each weeklong site visit, and ensure that the badge be worn in a visible location on the worker at all times while on the premises of the VA property. It is the responsibility of the contractor to park in the appropriate designated parking areas. Information on parking is available from the VA Police. The VA will not invalidate or make reimbursement for parking violations of the contractor under any conditions. Smoking is prohibited inside any buildings at the VA. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of VA regulations may result in a citation answerable in the United States District Court. Infection Control: There is a potential for exposure to blood borne or other infectious material with equipment throughout the hospital. All contractor personnel are cautioned and must use "Universal Precautions" (i.e. hand washing, wearing protective gloves, aprons, and goggles, etc.) as appropriate during the performance of this contract. Contractor shall provide HEPA vacuums, plastic sheeting and any other materials needed to prevent infection from construction dust due to penetrations. Payment: Payment will be made at completion of service and upon receipt of a properly prepared invoice which references the period for which payment is due, the contract number and/or assigned purchase order number. The invoice must include the correct payment address and direct debit account information. Personnel Qualifications: Contractor personnel performing under this contract will be fully qualified and competent for the equipment to be performed Service Quality: All services provided in this contract must meet manufacturers' performance and technical specifications, Federal Regulations, VA Regulations, and meet the requirements of The Joint Commission and Life Safety Code. The contractor shall document all maintenance and provide said documentation to the COR upon completion of each service action. PERIOD OF PERFORMANCE: The period of performance is for the extent of the service provided. Upon completion and shall not last for a period of more than 14 working days.
