Description
This amendment is to add a FAR Clause, extend the due date for quotes, and to provide answers to additional questions asked. 1. Add the following FAR Clause: FAR Clause 52.208-9 Contractor Use of Mandatory Sources of Supply or Services 2. The due date for quotes is hereby extended to Monday, June 25, 2018 by 1400 (2:00pm). Quotes are to be received via email to donna.sizemore@va.gov. 2. Answers to Questions Received in regard to the Combined Solicitation/Synopsis: Q6 Are there any costs to the contractor for the badging process? A6 No, other than the individuals time in going to the badging office. If so can you provide the cost of the badging process? N/A Q7 Is the contractor responsible for cleaning and restroom supplies in the guest bathrooms as well? A7 Yes, paper towels, toilet paper and soap for the hand dispenser (which is just a pump dispenser and refillable like at your home - not on the wall). This bathroom is not highly used, mostly just visitors to the house (guests that are staying there typically use the bathroom in their room). Q8 If so what exactly are all the supplies needed for the guest restrooms? A8 As mentioned in the contract Performance Work Statement, the paper supplies are to be more of a "residential quality" vs. commercial /hospital grade (Toilet Paper/Paper Towels should not be paper thin) Q9 The pricing breakdown section is asking for a supply and equipment cost but does not have a line item for the supply and equipment cost, only for labor, health & welfare, management and sick time. Where do we input put the cost of supplies? A9 Add a line for cost of supply and equipment Q10 Will the drawings provide the square footage of carpet verses tile flooring? A10 The customer was just made aware that the upstairs will be all wood flooring. The kitchen, dining area, laundry room and all hallways are a ceramic tile. The common areas of the living room and family room are large medium pile carpets. The main staircase is wood and has a secured runner. The downstairs bedrooms will likely be wood (changes to the finish schedule/work have been / are being made). I just saw the marble thresholds that will be between the bathroom and bedroom in each of the suites. Bathrooms all are ceramic tile, showers are tiled as well. Q11 And will it provide the other flooring types throughout the facility with square footages? A11 Customer does not have square footage details. See A10 for description if various rooms and flooring type. The drawings provide details as to length and width. Q12 Can you provide what supplies exactly that the contractor is responsible for? A12 All cleaning supplies needed to clean common areas and guestrooms: including supplies to clean bathroom, kitchen, floors, tables, countertops, and windows. Mops, towels for cleaning, buckets, vacuum, etc. Need to provide paper towels and toilet paper for all guestrooms, managers office, kitchen and public bathroom. Q13 Who is responsible for bed bug infestation, treatment and removal of bed bugs? A13 The actual infestation/removal, will be Dayton VA Medical Center. The housekeeping contract would help clean the room following infestation. Q14 How many public restrooms are there?   A14 One, downstairs, and only has a toilet and a sink. Q15 Can you provide the number of restrooms, along with how many dispensers and what type of dispenser will be installed? This is for supply costs. A15 There are 16 suites all with a private bathroom, one public restroom, and one restroom in the manager s office (toilet, sink and shower, the shower is rarely used). There will be decorative hand soap dispensers (you can fill with large jug liquid hand soap). Do not need to supply laundry supplies, dish soap or dishwasher soap. Q16 Is this a one story building? A16 2 story with an elevator. 8 suites are downstairs (4 on each wing) and 4 suites are upstairs. There are 2 internal staircases (one goes directly outside), one main staircase and an elevator to access the upstairs.
