- Draft SOW This is a Small Business Sources Sought notice. This is NOT a solicitation for proposals, proposal abstracts, or quotations. The Center for Disease Control and Prevention (CDC), National Institute for Occupational Safety and Health (NIOSH), is seeking information on a potential requirement for "Medical Services - NIOSH Morgantown." The purpose of this notice is to obtain information regarding: (1) the availability and capability of qualified small business sources; (2) whether they are small businesses; HUBZone small businesses; service-disabled, veteran-owned small businesses; 8(a) small businesses; veteran-owned small businesses; woman-owned small businesses; or small disadvantaged businesses; and (3) their size classification relative to the NAICS code 621999 for the potential acquisition. See Draft Sample Work Statement. 1 Background and Need - Physicals at the National Institute of Occupational Safety and Health (NIOSH) are provided to the Health Hazard Evaluation Field Team members, Public Health Service Commissioned Officers, Safety Office personnel and Facilities Maintenance personnel. All other employees have the opportunity to participate in the medical monitoring program. Those employees who elect to participate in the program are given physicals based on their age. NIOSH has a requirement for an estimated 135 Physical Examinations (i.e., physicals) per year. Additional ad hoc testing, ad hoc vaccinations, and supplemental occupational health services as described below are required. NIOSH has employees that are part of the CDC Emergency Response Team as well as the PHS Commissioned Corps Readiness Team. These teams are involved in incident management activities to include the prevention of, preparedness for, response to, and recovery from terrorism, major natural disasters, and other major emergencies. Depending on the type and magnitude of the emergency, additional NIOSH employees not currently on the team may be called to respond to an emergency. This would require immediate medical services which would include a responder readiness (general fit for duty) medical clearance and may also include immunizations and/or respiratory medical clearance. It is essential that the contractor administering the medical surveillance program for the NIOSH-Morgantown facility be located nearby to the facility (within approximately 20 miles) or be able to provide services in that vicinity within eight (8) hours of request for services. 2 Project Objective - The following objectives are established for the contractor to provide the minimum services: - Employee physicals - Testing - Medical monitoring and surveillance - Monitored care when appropriate - Records and Data Management - Support as required for the emergency and disaster preparedness team - Other occupational medical services described herein - Supporting transition between medical services providers - Additional Services as described herein. Anticipated period of performance. Base of one (1) year with four (4), one (1) year options. Capability statement /information sought. Respondents who believe that they possess the expertise and experience to meet the government's needs as described above are invited to submit technical and management capabilities addressing those requirements. These may include but are not limited to: (a) staff expertise, including their availability, experience, and formal and other training; (b) current in-house capability and capacity to perform the work; (c) prior completed projects of similar nature; (d) corporate experience and management capability; and (e) examples of prior completed Government contracts, references, and other related information. Respondent must indicate in your capability statement: - Organization name, - Address, - DUNS number, - Business size Classification (e.g., 8(a), HUBZone, etc.) pursuant to the applicable NAICS code of 621999-All Other Ambulatory Health Services size standard which is $15 million. - Points of contact, - Email, - Telephone number, Page Limitation: No more than five (5) pages, 8 ½ x 11.
