TITLE: R- NOTICE OF INTENT TO AWARD SOLE SOURCE SOL: 3624218Q9524 DUE: 26 June, 2018 SET ASIDE: N/A NAICS: 611699 POP STATE: 7/1/2018 AGENCY: Department of Veterans Affairs - NYC & SA POC: Carlton Lunsford, email: carlton.lunsford@va.gov DESCRIPTION: NOTICE OF INTENT TO AWARD A SOLE SOURCE CONTRACT TO Evacusled Inc. Market research was conducted and determined that Evacusled Inc is the only vendor capable of providing the Evacusled Training service necessary that meet the requirements. All items are proprietary to Evacusled Inc and not able to be sourced out to another. This Notice of Intent is not a request for Proposals or Quotations. No solicitation document exists and no telephone or email inquiries will be accepted. No contract award will be made on the basis of proposals, or quotations received in response to this notice. The government MAY consider all Expressions of Interest (EOI) received prior to 27 June, 2018". A determination by the government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement. Responses to this notice must be emailed to carlton.lunsford@va.gov and received by 10:00AM EST on 27 June, 2018. Submitters are responsible for ensuring their EOI reaches the designated contracting officer on-time. See attached Statement of Work below for additional details. STATEMENT OF WORK GENERAL INFORMATION 1. Title of Project: Evacusled Training for Nursing and EMS 2. Scope of Work: The contractor shall provide all resources necessary to accomplish the deliverables described in this statement of work (SOW), except as may otherwise be specified. This is for re-training of staff at the New York and St. Albans campuses in the use of Evacusleds, which are used to evacuate immobile patients down the stairs in emergencies when elevators cannot be used. 3. Background: Evacusled vendor will conduct the training during fy18 to Nursing and EMS on the deployment and horizontal/vertical movement of evacusleds. Re-training of staff is required as an Action Plan in response to a recommendation from the recent CARF survey of NY- PM&R. This training will also allow us to meet several VHA Office of Emergency Management EMCAP capabilities/assessment criteria (2.3.5: The facility staff are trained on the use of available evacuation equipment; and 2.3.6: The facility conducts evacuation exercises in accordance with the facility plans, policies, and accreditation requirements) and will help us meet Joint Commission standard EM 02.02.07, EP 7: As part of its Emergency Operations Plan, the hospital prepares for how it will manage staff during an emergency. The hospital trains staff for their assigned emergency response roles. Training is NOT needed at the Brooklyn campus, as we will be replacing our Evacusleds with an alternative device and training on the new device is included in the purchase. Note this request must be SOLE-SOURCED as only the vendor who provided the Evacusleds can provide training on the use of this equipment. The last time this training was provided was in fy 2014. 4. Performance Period: The contractor shall complete the work required under this SOW in 60 calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor s proposal, the contractor s proposed completion date shall prevail. 5. Type of Contract: Firm-Fixed-Price. STATEMENT OF WORK Evacusled will provide a total of 9 one-hour sessions in the placement of patients into Evacusleds (Nursing), horizontal transport down the hall (Nursing), vertical transport down stairs (Nursing and EMS), and repacking of Evacusleds after use (Nursing). These sessions to be divided between the New York and St. Albans campuses. Response date (deadline): Training must be completed in fiscal year 2018.
