Description

03 I NCIC Consent Form Instructions. 03 F NCIC Consent Form - Dec 2016. 02 I MOTCO Form 190-3 Instructions. 02 F 190-3-R for Pier 3 Pile Wrapping Project spreadsheet. 01 Supplemental USACE Instructions. The Government anticipates issuing a Request for Proposal (RFP) with the intent of awarding a Firm-Fixed Price construction contract for the Pier 3 Pile Wrapping and Deck Repairs at Military Ocean Terminal Concord (MOTCO), Concord, CA. Project Description: The Contractor will furnish all labor, materials, equipment, transportation and supplies necessary to fulfill the terms and conditions stated in this contract. The Contractor will be responsible for all costs related to Pier 3 repairs at Military Ocean Terminal Concord. Repairs include but are not limited to wrapping piles underwater while being sensitive to the environment and aware of potential existence of MEC/UXO (will need to complete MEC/UXO clearance prior to wrapping piles). Key characteristics of this project include: a. Water depth less than 60 feet. b. Water temperature less than 60 degrees F. c. Water velocity up to 3 knots, based on tidal current prediction for the channel near the site. d. Visibility less than 2 feet, and often times zero, based on previous diving work at the site. e. Hazards: Overhead, buried MEC/UXO and other objects below mud line. f. Very soft bottom (mud) and shallow water (depth less than 8 feet). In accordance with the DFARS 236.204(i), the magnitude of this construction project is between $10,000,000 and $25,000,000. The RFP is estimated to be issued in July 2018 with proposals due 30 days later. The RFP will be issued on the Government Point of Entry known as FedBizOpps (www.fbo.gov) and will establish a firm proposal due date/time. The site visit/pre-proposal conference will be held on 19 July 2018 at 0900. All participants will meet at 5110 Port Chicago Highway, Concord, CA, 94520. SPECIAL ACCESS REQUIREMENTS: Companies must submit one 190-3 form with all participants as well as NCIC consent forms for each individual attending the site visit. These forms must be received by 9 July at 2:00 PM. Submit the completed forms to: Jaime.S.Middlebrooks@usace.army.mil with a courtesy copy to Daniel.E.Czech@usace.army.mil and Tracy.N.Tenholder@usace.army.mil.
