- Page 1 of 3 The Department of Veterans Affairs Medical Center, Network Contracting Office (NCO) 9, on behalf of Veterans Integrated Service Network (VISN) 9 Health Care System, is issuing this source sought synopsis as a means of conducting market research to identify SDVOSB manufacturers having an interest in and the resources to support this requirement for a firm-fixed priced indefinite delivery requirement (ID/R) contract for prescription eyeglasses. The result of this market research will contribute to determining the method of procurement. The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 339115. THERE IS NO SOLICITATION AT THIS TIME. This request for capability information does not constitute a request for proposals or quotes; submission of any information in response to this market survey is purely voluntary; the government assumes no financial responsibility for any costs incurred. The government will evaluate market information to ascertain potential market capacity to: 1. Determine if a small business qualify to provide manufactured products or other supply items under SDVOSB set-aside in accordance with 13 C.F.R. §121.406. 2. Provide services consistent in scope and scale with those described in this notice and otherwise anticipated. Secure and apply the full range of corporate financial, human capital, and technical resources required to successfully perform similar requirements. 3. Implement a successful plan that includes: compliance with program schedules; cost containment; meeting and tracking performance. 4. Provide supplies or services under a firm-fixed price indefinite delivery requirements contract. Performance locations: All work performed will be performed at each VA facility. Provision of space for the Dispensing Optician will be provided by each VA facility. The space requirements including phones are the responsibility of the VA. VA will provide desk, chair and waiting chairs for customers and space to store forms and supplies. VA will provide computer terminal, keyboard and mouse. Contractor to provide prescription eyeglasses according to the contract specifications for beneficiaries of the Department of Veterans Affairs Centers in VISN 9 which presently consists of: VAMC CBOC Lexington, KY Somerset, KY Louisville, KY Chattanooga, TN Memphis, TN Clarksville, TN Mountain Home, TN Jackson, TN Murfreesboro, TN Knoxville, TN Nashville, TN Nonconnah, TN If your organization has the potential capacity to provide the following: CLIN/Items: Estimated 56,311 pairs of eyeglasses annually to eligible veterans. Contractor shall provide frame selection for men, women and unisex including a mix of metals with plastic, and spring hinges. Frame sizes shall include large frame sizes (54-60), short bow lengths (125 or 130), and a frame selection variety for all age groups (21-100). Contractor shall provide single vision, bifocal, progressive bifocal, trifocal, progressive, progressive addition, special, high index, tinted and scratch resistant lenses available in glass, plastic, polycarbonate, and high index plastic; when lens blanks are available. The contractor shall provide prescription eyeglasses to eligible veterans. All Lenses shall be fabricated and provided in accordance with the standards set forth in the current edition of American National Standards for Prescription Ophthalmic Lenses - Recommendation ANSI Z80.1 or latest edition. All ophthalmic lenses shall be in accordance with Food and Drug Administration (FDA) Regulation (2) CFR, Parts 3 and 4 for impact resistance, unless otherwise specified in the order. All frames shall be provided in accordance with the current edition of the American National Standard Requirements for Dress Ophthalmic Frames ANSI Z80.5 or latest edition. The contractor shall supply a licensed Dispensing Optician/On-Site Fitter to each VA Medical Center/facility in VISN 9, five days a week, Monday through Friday, 40 hours a week, excluding federal holidays. If a State does not require an American Board Certified Dispensing Optician, the Dispensing Optician must abide by the State Licensing Requirements. Proof of State licensing requirements must be provided. The Dispensing Optician will take all necessary facial and frame measurements to order glasses as per doctor's prescription using the VHA VISTA CPRS electronic consult package. Measurements and refraction data may be entered this consult package; information entered in the electronic consult package will then be forwarded electronically to the facility Prosthetic Service. All employees assigned as fitters will complete annual training required by VHA policy including privacy/HIPPA training, cyber security, privacy awareness training and any other training specified at contractor expense via the internet. This includes a background security check initiated before performing any work. This applies to all contract employees requiring any type of IT access. This includes all fill in or backup personnel. The contractor shall complete the training and complete a Statement of Commitment and Understanding, certifying completion of the training and understanding of their commitment to protect sensitive VA information obtained as a result of performance of the contract for each individual offered under this contract. The contractor must possess all licenses, permits, accreditations and certificates as required by law. Please provide the following information: 1. Under which NAICS code does your company usually provide the requirements described in items/description list? Please provide rationale for your answer if different from NAICS 339115. 5. Tailored capability statements addressing the particulars of this effort, with appropriate documentation supporting claims of organizational and staff capability.
