- Bid Form Wage Determination Drawings Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR), subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; this is a Request for Quotation (RFQ) and a written solicitation will not be issued. Funds are not presently available for this contract. The Government's obligation under this contract is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise until funds are made available to the Contracting Officer for this contract and until the Contractor receives notice of such availability, to be confirmed in writing by the Contracting Officer. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-98. The NAICS is 238330, Flooring Contractors. The Small Business Size Standard for this NAICS code is $15M. This acquisition shall be available as a total small business set aside. Period of Performance: 30 Days Description: Apply Tennant Eco-PT 250 Power-Trowel Epoxy Overlay to the Building 33 shop floor in accordance with the drawings provided. (Rooms: 113, 114, 114A 115 and 116). Contractor must perform all work described in the manufacturers specifications regarding surface preparation, cleaning and application. Overlay is to be light gray in color, anti-skid, resistant to petroleum products and stain resistant. The final product will cover all previous damage and repairs, providing the appearance of a new floor. The overlay shall be no thinner than 3/16 inches thick. Additionally, the contractor will paint clear zones on floor as follows: The floor area around permanent and self-contained eyewash stations will be painted green with white stripes. The zone will provide 36 inches of clearance around the device. The floor area around all power panels, circuit breaker panels and fire extinguishers will be painted with a three inch yellow-and-black hashed line providing 18 X 36 inch clear zone in front and to the side device. Paint a 3 inch red line across welding shop entry. Paint red wheel tick marks for alignment computer adjacent to Rm 113 Door 13.184th Vehicle Maintenance will move all equipment, tooling or shelving NOT secured to the floor. Any equipment, tooling or shelving that IS secured to the floor or is too large or bulky to move, will be masked off and painted around. CLIN Description 0001 Bldg. 33 Maint. Epoxy Floor Installation 0002 Contract Manpower Reporting Application List of Attachments: a) Attachment I, Statement of Work, 21 June 2018 b) Attachment II, Drawings, 24 May 2018 c) Attachment III, Service Contract Act Labor Rates, 4 June 2018 d) Attachment IV, Bid Sheet A site visit shall be arranged by request. All requests for a site visit shall be submitted by 5 July 2018 at 1:00 PM CST. All contractors interested in attending shall submit the following information: - Full name (EXACTLY as it appears on the ID) - Birth Date - Social Security Account Number (SSAN) - Two (2) forms of credentialed ID for each employee being utilized for on- site work (both of which must be photo). The following are acceptable: passport, state issued driver's license, state issued photo identification, Social Security card, or Base identification (only if issued from previous contract). - A photocopy of the ID's to be used. One of the forms of identification must have a SSAN on it. All questions are due by 5 July 2018, 10:00 AM CST to POC's listed below. Quotes must be valid through 30 September 2018. Quotes shall be provided on the attached bid sheet, signed, dated and submitted by 10 July 2018, 10:00 AM CST to both point of contacts listed below. The Offeror is responsible for ensuring the quote is received by the Government by the due date and time specified above.
