Replace Flooring B6 Corridor, Air Monitoring Project Number: 528A5-18-534 B6 Flooring Third Party Asbestos Air Monitoring WORK SCOPE Existing Conditions: The Canandaigua VAMC is located at 400 Fort Hill Avenue, Canandaigua, NY 14424. The layout is a campus with more than 30 buildings located on over 150 acres. Normal working hours are Monday Friday 8:00am 4:30pm. Building 6, ground floor corridor will have the existing flooring abated in a construction project to replace the flooring. The scope of this work is to provide project and air monitoring services for this project. Additionally, the contractor will take baseline samples for asbestos and mold and provide those to the VA. The VA will provide the baseline sample results to the remediation contractor. Scope of Work: The scope of this project is to provide a review of contractor certifications, compliance with regulations during abatement, air monitoring throughout the asbestos abatement and to provide summary report include clearance certification. Daily logs will be submitted along with all chains of custody as outlined in the regulations. The project will include air monitoring for abatement of the below approximate quantities: 2000 SF Asbestos tile with mastic The CIH will; assure quality; adherence to the specification; resolve problems; prevent the spread of contamination beyond the regulated area; take and provide results for background/initial sampling and assure clearance at the end of the project. In addition, work includes performing the final inspection and testing to determine whether the regulated area or building has been adequately decontaminated. All asbestos air monitoring is to be done utilizing PCM/TEM. The CIH will perform the following tasks for asbestos abatement monitoring. 1. Establish background levels before abatement begins by collecting background samples. Retain samples for possible TEM analysis. 2. Perform continuous air monitoring, inspection, and testing outside the regulated area during actual abatement work to detect any faults in the regulated area isolation and any adverse impact on the surroundings from regulated area activities. 3. Perform unannounced visits to spot check overall compliance of work with contract/specifications. These visits may include any inspection, monitoring, and testing inside and outside the regulated area and all aspects of the operation except personnel monitoring. 4. Provide support to the VA representative such as evaluation of submittals from the Contractor, resolution of conflicts, interpret data, etc. 5. Perform, in the presence of the VA representative, final inspection and testing of a decontaminated regulated area at the conclusion of the abatement to certify compliance with all regulations and VA requirements/specifications. 6. Issue certificate of decontamination for each regulated area. Code requirements: All abatement will be in compliance with NYSCR 56. The abatement will also be in accordance with all applicable codes including Veterans Affairs standards which can be found at https://www.cfm.va.gov/til/. This will include, but not be limited to: Department of Veterans Affairs Design Manuals, VA Publications, and Construction Standards, National Fire Protection Code (NFPA), Life Safety Code, Underwriters Laboratory (UL), International Building Code IBC 2003, National Electric Code (NEC), and the VA Information Security Handbook 6500.6. Occupational Health and Safety (OSHA), and Environmental Protection Agency (EPA) requirements (to include Mold Remediation in Schools and Commercial Buildings).Article 32 of the New York State Labor Law will be complied with. All work will be completed by certified individuals. Specifications: Included in the attached specifications applicable to this work are the following: Division 01, General Requirements Division 02, Existing Conditions Drawings: 100% project drawings General Requirements: Where incidental work is required to accomplish work shown on the drawings, this work is implied and shall be included as part of the base bid. Prior to any work commencing in an area, the contractor shall notify the COR of his intention to work in the area and shall schedule a walkthrough with the COR. The contractor shall receive all deliveries of materials, equipment, tools, or other goods off of station, and bring them to the job site. The Canandaigua VA shall not accept deliveries for project purposes. The contractor shall not proceed with any on-site work approval to proceed from the CO and COR. Submittals: Submittals are required for all items outlined in the specification and include, but are not limited to the following major submittals. The items highlighted are required to be submitted with your offer: Division Item 02 Staff organization chart showing all personnel who will be working on the project and their capacity/function. 02 Staff qualifications, training, accreditations, and licenses 02 Submit laboratory licensing 02 Proof of contractor licensing 02 State license for air monitoring 02 Copy of insurance policy 02 Daily logs 02 Documentation of inspection of the regulated area 02 Asbestos completion/clearance submittals 02 Worker records (02 85 00.00 20 - section 1.4.1.4) Evaluation: You will be rated for your technical capabilities on the factors below. In the event of a tie, the firm with the lowest priced offer will be accepted. Staff qualifications, training, accreditations, and licenses Proof of contractor licensing State license for air monitoring Anticipated Project Duration: The total projected duration for these monitoring services is 30 days. Off hours work may be required on a case by case basis. The contractor will need to be prepared to start monitoring within one week of notice of abatement start. The contractor will need to be able to remobilize as is necessary to support throughout the project. Security: 1. Citations to pertinent laws, codes and regulations such as 44 U.S.C Chapter 21, 29, 31 and 33; Freedom of Information Act (5 U.S.C. 552); Privacy Act (5 U.S.C. 552a); 36 CFR Part 1222 and Part 1228. 2. Contractor shall treat all deliverables under the contract as the property of the U.S. Government for which the Government Agency shall have unlimited rights to use, dispose of, or disclose such data contained therein as it determines to be in the public interest. 3. Contractor shall not create or maintain any records that are not specifically tied to or authorized by the contract using Government IT equipment and/or Government records. 4. Contractor shall not retain, use, sell, or disseminate copies of any deliverable that contains information covered by the Privacy Act of 1974 or that which is generally protected by the Freedom of Information Act. 5. Contractor shall not create or maintain any records containing any Government Agency records that are not specifically tied to or authorized by the contract. 6. The Government Agency owns the rights to all data/records produced as part of this contract. 7. The Government Agency owns the rights to all electronic information (electronic data, electronic information systems, electronic databases, etc.) and all supporting documentation created as part of this contract. Contractor must deliver sufficient technical documentation with all data deliverables to permit the agency to use the data. 8. Contractor agrees to comply with Federal and Agency records management policies, including those policies associated with the safeguarding of records covered by the Privacy Act of 1974. These policies include the preservation of all records created or received regardless of format [paper, electronic, etc.] or mode of transmission [e-mail, fax, etc.] or state of completion [draft, final, etc.]. 9. No disposition of documents will be allowed without the prior written consent of the Contracting Officer. The Agency and its contractors are responsible for preventing the alienation or unauthorized destruction of records, including all forms of mutilation. Willful and unlawful destruction, damage or alienation of Federal records is subject to the fines and penalties imposed by 18 U.S.C. 2701. Records may not be removed from the legal custody of the Agency or destroyed without regard to the provisions of the agency records schedules. 10. Contractor is required to obtain the Contracting Officer's approval prior to engaging in any contractual relationship (sub-contractor) in support of this contract requiring the disclosure of information, documentary material and/or records generated under or relating to this contract. The Contractor (and any sub-contractor) is required to abide by Government and Agency guidance for protecting sensitive and proprietary information.
